Comparison on the application between Dexmedetomidine and Lidocaine in bronchoscopy for patients undergoing tracheal intubation and mechanical ventilation

Abstract:Objective To compare the application between Dexmedetomidine and Lidocaine in bronchoscopy for patients undergoing tracheal intubation and mechanical ventilation.Methods A total of 90 patients with pneumonia underwent tracheal intubation and mechanical ventilation in Hexian Memorial Hospital of Panyu District of Guangzhou City from January 2019 to February 2020 were selected as the research objects,they were divided into the control group (45 cases) and the experimental group (45 cases) according to random number table method.During bronchoscopy,the control group was treated with traditional oxygen-driven Lidocaine atomization surface anesthesia method,while the experimental group was handled with Dexmedetomidine intravenous anesthesia method.The changes of blood pressure,heart rate,finger pulse oxygen at before bronchoscopy (T0),during bronchoscopy (T1) and half an hour after bronchoscopy (T2) and anesthesia effect,the total detection rate of bronchoalveolar lavage fluid culture were compared between the two groups.Results The excellent and good rate of anesthesia in the experimental group was 100.0%,which was higher than 44.4% in the control group,and the difference was statistically significant (P<0.05).The systolic blood pressure and heart rate of the experimental group at T1 were lower than those of the control group,and the differences were statistically significant (P<0.05).There was no significant difference in systolic blood pressure and heart rate at T0 and T2 between the two groups (P>0.05).The systolic blood pressure and heart rate of the control group at T1 were higher than those at T0,and the differences were statistically significant (P<0.05).There were no significant differences in systolic blood pressure and heart rate in the experimental group at T1 and T2 compared with T0 (P>0.05).The total detection rate of bronchoalveolar lavage fluid culture of the experimental group was 71.1%,which was higher than 46.6%of the control group,and the difference was statistically significant (P<0.05).There were no significant differences in the detection rates of Carbapenem-resistant Acinetobacter baumannii and Fungi between the two groups (P>0.05).Conclusion For conscious patients with pneumonia in ICU undergoing tracheal intubation and mechanical ventilation,the influence on vital signs of Dexmedetomidine intravenous general anesthesia is less than that of Lidocaine atomized surface anesthesia,and the anesthesia effect is better than that of Lidocaine atomized surface anesthesia,and which has less influence on the results of bronchoalveolar lavage fluid culture.
