超声引导下星状神经节置管连续阻滞治疗头颈部带状疱疹后神经痛的临床效果

摘要目的探讨超声引导下星状神经节置管连续阻滞治疗头颈部带状疱疹后神经痛（PHN）的临床效果。方法选择2018年1月～2019年12月江西省九江市中医医院收治的80 例PHN 患者，按随机数字表法分为观察组和对照组，每组各40 例。对照组实施常规药物治疗，观察组实施常规药物治疗联合星状神经节置管连续阻滞，比较两组治疗前及治疗后1、7、14 及28 d 的视觉模拟量表（VAS）评分和情绪评分（ES），评价两组的改善情况。结果观察组治疗后7、14 和28 d 的VAS 和ES 评分低于对照组，差异有统计学意义（P<0.05）；观察组的治疗总优良率为92.5%，高于对照组的77.5%，差异有统计学意义（P<0.05）。结论药物联合超声引导下星状神经节置管连续阻滞治疗PHN 的效果优于单纯西药治疗，值得临床推广。

Abstract：Objective To investigate the clinical effect of ultrasound-guided continuous stellate ganglion catheter block in the treatment of post-herpetic neuralgia （PHN）. Methods A total of 80 patients with head and neck PHN admitted to Jiujiang Hospital of Traditional Chinese Medicine, Jiangxi Province from January 2018 to December 2019 were selected and were divided into observation group and control group according to the random number table method, 40 cases in each group. The control group was treated with conventional drugs, and the observation group was treated with conventional drugs combined with continuous stellate ganglion catheterization. The visual analogue scale (VAS) and emotional score (ES) of the two groups of patients before and at 1, 7, 14, and 28 days after treatment were compared.The improvement of patients was evaluated. Results The VAS and ES scores of the observation group were lower than those of the control group at 7, 14 and 28 days after treatment, the differences were statistically significant (P<0.05).The excellent and good rate of the observation group was 92.5%, which was higher than that of the control group of 77.5%, the difference was statistically significant (P<0.05). Conclusion The combined effect of drugs combined with ultrasound-guided continuous stellate ganglion catheter for the treatment of PHN is better than Western medicine alone, and it is worthy of clinical promotion.
