Clinical effect of ultrasound-guided femoral nerve block combined with lateral femoral cutaneous nerve block for patella fracture surgery in elderly patients

Abstract:Objective To investigate the clinical effect of ultrasound-guided femoral nerve block combined with lateral femoral cutaneous nerve block for patella fracture surgery in elderly patients.Methods A total of 100 elderly patients with patella fractures who were admitted to Guangxin District People′s Hospital from October 2018 to March 2020 were selected as the research objects.According to the random number table method,they were divided into combined spinal-epidural anesthesia group(L group)and combined ultrasound-guided femoral nerve block Lateral femoral cutaneous nerve block group(N group),with 50 cases in each group.The hemodynamic indexes(diastolic blood pressure [DBP],systolic blood pressure [SBP],heart rate [HR])in the two groups at the time of anesthesia entry(T0),at the end of anesthesia(T1),at the beginning of surgery(T2)and at the end of surgery(T3)were compared.The total incidence of postoperative complications in the two groups was compared.Results The DBP and SBP at T1,T2 and T3 of the two groups were lower than those at T0,but the DBP and SBP at T1,T2,and T3 of the N group were higher than those of the L group,the difference in the interaction ratio between groups,time points,and between groups and time points was statistically significant(P<0.05).At T1,T2 and T3,HR in both groups was lower than that in T0,and the difference at different time points was statistically significant(P<0.05).There was no significant difference between groups,between groups and time point(P>0.05).The total incidence of complications in group N was lower than that in group L,and the difference was statistically significant(P<0.05).Conclusion The use of ultrasound-guided femoral nerve block combined with lateral femoral cutaneous nerve block in elderly patella fracture surgery can effectively control hemodynamics,keep vital signs relatively stable,enhance the analgesic effect and have high safety.
