针灸联合推拿治疗对腰椎间盘突出症患者疼痛、功能受限及生活质量的影响

摘要目的探讨针灸联合推拿治疗腰椎间盘突出症（LDH）的疼痛效果及其对功能受限和生活质量的影响。方法选取2016年1月～2019年1月于我院就诊的84 例LDH 患者作为研究对象，按照随机数字表法分为对照组和观察组，每组各42 例。对照组患者行针灸治疗，观察组患者行针灸联合推拿治疗，两组患者均连续治疗1 个月，观察两组患者的临床疗效、疼痛、功能受限、生活质量和不良反应发生情况。结果观察组患者的临床总有效率为95.24%，高于对照组的73.81%，差异有统计学意义（P＜0.05）。治疗后，观察组患者的视觉模拟量表评分及Oswestry 功能障碍指数均低于对照组，差异有统计学意义（P＜0.05）；观察组患者的腰部活动度评分及SF-36 生活质量调查表评分高于对照组，差异有统计学意义（P＜0.05）；观察组患者的自觉疼痛缓解起效时间短于对照组，镇痛持续时间长于对照组，差异有统计学意义（P＜0.05）。两组患者的不良反应总发生率比较，差异无统计学意义（P＞0.05）。结论针灸联合推拿治疗LDH 的临床效果显著，安全性良好，能有效缓解患者的疼痛和功能受限等症状，提升患者的生活质量。

Abstract：Objective To explore the effect of acupuncture combined with massage in the treatment of lumbar disc herniation (LDH) pain and its influence on functional limitation and quality of life. Methods A total of 84 patients with LDH who were treated in our hospital from January to September 2019 were selected as subjects, and were divided into the control group and the observation group according to the random number table method, 42 cases in each group. The control group was treated with acupuncture. The observation group was treated with acupuncture combined with massage. The clinical efficacy, pain, function limitation, quality of life and adverse reactions were observed. Results The total clinical effective rate in the observation group was 95.24%, which was higher than that in the control group of 73.81%, the difference was statistically significant (P＜0.05). After treatment, the visual analogue scale scores and Oswestry dysfunction indexes in the observation group were lower than those in the control group, and the differences were statistically significant (P＜0.05). Waist activity and SF-36 quality of life questionnaire scores were higher than those in the control group, the differences were statistically significant (P＜0.05). The effect time of conscious pain relief in the observation group was shorter than that in the control group, and the duration of analgesia was longer than that in the control group, with statistically significant differences (P＜0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P＞0.05). Conclusion The clinical effect of acupuncture combined with massage for the treatment of LDH is remarkable, and the safety is good. It can effectively alleviate the symptoms such as pain and function limitation of patients and improve the quality of life of patients.
