高压氧联合调强放射治疗中晚期鼻咽癌患者的效果

摘要目的探讨高压氧（HBO）联合调强放射治疗中晚期鼻咽癌患者的效果。方法选取解放军南部战区海军第一医院2016年6月～2018年6月收治的80例中晚期鼻咽癌患者作为研究对象，按照患者治理意愿分为HBO联合组（34例）与单纯放疗组（46例）。HBO联合组先进行HBO治疗再进行调强放射治疗，单纯放疗组采用常规调强放射治疗方法。完成治疗后，观察并记录两组的体征症状、卡氏功能状态评分及放疗急性不良反应，比较鼻咽部肿瘤体积大小变化情况。结果治疗完成后，HBO联合组患者的神经系统症状和体征的缓解率高于单纯放疗组，差异有统计学意义（P＜0.05）；HBO联合组患者的生活质量改善效果优于单纯放疗组，差异有统计学意义（P＜0.05）；HBO联合组3级及以上急性放射性不良反应的发生率（17.6%）低于单纯放疗组（41.3%），差异有统计学意义（P＜0.05）；患者放疗结束后1个月复查MRI，HBO联合组患者的肿瘤治疗缓解率高于单纯放疗组患者，差异有统计学意义（P＜0.05）。结论对于中晚期鼻咽癌患者，HBO联合调强放疗可以提高调强放疗疗效，减轻神经系统症状和体征，降低急性放射性不良反应的发生率，改善患者生活质量。

Abstract：Objective To investigate the efficacy of hyperbaric oxygen (HBO) combined with intensity modulated radiotherapy (IMRT) on patients with advanced nasopharyngeal carcinoma (NPC).Methods A total of 80 patients with advanced NPC from June 2016 to June 2018 in the First Hospital of Naval Force of Southern Theater Command PLA were selected as the research object.The patients were divided into the HBO combined with IMRT group (34 cases)and the only IMRT group (46 cases) according to their treatment intention.In the HBO combined with IMRT group,HBO treatment was performed first,then intensity-modulated radiotherapy.The only IMRT group was treated with normal IMRT.After the treatment,the patients′ symptoms and signs,Karnofsky performance state score and acute radiotherapy response were observed and recorded in the two groups,and the nasopharyngeal tumor volume regression was compared between the two groups.Results After the completion of the whole treatment course,the regression rate of neurological symptoms and signs of patients in the HBO combined with IMRT group were higher than those in the only IMRT group,and the difference was also statistically significant (P＜0.05).The improvement effect of quality of life in the HBO combined with IMRT group was better than that in the only IMRT group,with statistically significant differences (P＜0.05).The incidence of acute radioactive side reactions equal or greater than grade 3 in the HBO combined with IMRT group was 17.6%,which was much lower than that in the only IMRT group (41.3%),and there was also a statistically significant difference (P＜0.05).MRI was reexamined 1 month after radiotherapy,and the tumor remission rate of patients in the HBO combined with IMRT group was higher than that in the only IMRT group,and the difference was also statistically significant (P＜0.05).Conclusion For the patients with advanced NPC,HBO combined with IMRT can improve the curative effect of radiotherapy,relieve the neurological symptoms and signs,reduce the incidence of acute side effects of radiotherapy,and improve the quality of life of patients.
