外痔切除联合弹力线内痔套扎术治疗Ⅲ、Ⅳ度混合痔的临床效果

摘要目的探讨外痔切除联合弹力线内痔套扎术治疗Ⅲ、Ⅳ度混合痔的临床效果。方法选取2018年1～12月抚州市中医院收治的70 例Ⅲ、Ⅳ度混合痔患者作为研究对象，采用电脑盲选法将其分为观察组（n=35）与对照组（n=35）。对照组患者采用混合痔外剥内扎术治疗，观察组患者采用外痔切除联合弹力线内痔套扎术治疗。比较两组患者的手术时间、术中出血量、住院时间、相关症状评分、疾病复发率、并发症总发生率及治疗总有效率。结果观察组患者的手术时间、住院时间、创面愈合时间短于对照组，差异有统计学意义（P＜0.05）。观察组患者的术中出血量少于对照组，治疗总有效率高于对照组，差异有统计学意义（P＜0.05）。治疗前，两组患者的疼痛、水肿、便血症状评分比较，差异无统计学意义（P＞0.05）；治疗后，观察组患者的疼痛、水肿、便血症状评分低于对照组，差异有统计学意义（P＜0.05）。观察组患者的并发症总发生率、疾病复发率与对照组比较，差异无统计学意义（P＞0.05）。结论采用外痔切除联合弹力线内痔套扎术治疗Ⅲ、Ⅳ度混合痔，可明显改善或消除各项临床病症，提高手术治疗效率，且效果稳定，值得临床应用。

Abstract：Objective To investigate the clinical effect of external hemorrhoidectomy combined with elastic line internal hemorrhoid ligation in the treatment of mixed sputum of grade Ⅲand Ⅳ.Methods Seventy patients with mixed sputum of grade Ⅲand Ⅳwho were admitted to Fuzhou Hospital of Traditional Chinese Medicine from January to December 2018 were selected as subjects.They were divided into observation group(n=35)and control group(n=35)by computer blind selection method.The control group was treated with mixed external hemorrhoids and internal ligation.The observation group was treated with external hemorrhoidectomy and elastic line internal hemorrhoid ligation.The operation time,intraoperative blood loss,hospitalization time,symptom score,recurrence rate,total complication rate and total treatment efficiency of the two groups were compared.Results The operation time,hospitalization time and wound healing time of the observation group were shorter than those of the control group,and the differences were statistically significant(P＜0.05).The intraoperative blood loss in the observation group was less than that in the control group,the total effective rate was higher than that of the control group,and the differences were statistically significant(P＜0.05).Before treatment,the symptomscores of pain,edema and blood in the stool in the two groups showed no significant differences(P＞0.05).After treatment,the symptom scores of pain,edema and blood in the stool in the observation group were lower than those in the control group,with statistically significant differences(P＜0.05).The total incidence of complications and recurrence rate in the observation group were not significantly different from those in the control group(P＞0.05).Conclusion The treatment of mixed hemorrhoids of grade Ⅲand Ⅳwith external hemorrhoidectomy and elastic line internal hemorrhoid ligation can significantly improve or eliminate various clinical conditions,improve the efficiency of surgical treatment,and the effect is stable.It is worthy of clinical application.
