甲钴胺联合黄芪桂枝汤治疗糖尿病足的效果

摘要目的探讨甲钴胺联合黄芪桂枝汤治疗糖尿病足的效果及对微炎症状态的影响。方法选择2018年1月～2019年3月我院收治的84 例糖尿病足患者为研究对象，根据随机数字表法将其分为观察组与对照组，每组各42 例。对照组患者接受基础治疗及甲钴胺治疗，观察组在此基础上联合应用黄芪桂枝汤治疗，疗程为4 周。比较两组患者足部症状改善时间，临床疗效以及治疗前后空腹血糖（FPG）、餐后2 h 血糖（2 h PG）、糖化血红蛋白（HbA1c）及血清肿瘤坏死因子-α（TNF-α）、超敏C 反应蛋白（hs-CRP）水平的变化。结果治疗后，观察组患者FPG、2 h PG及HbA1c 分别为（5.73±1.14）、（8.07±1.53）mmol/L、（6.21±0.84）%，低于对照组的（6.23±0.74）、（9.27±0.93）mmol/L、（6.92±0.62）%，差异均有统计学意义（P＜0.05）；治疗后，观察组患者血清TNF-α、hs-CRP 分别为（9.54±2.32）ng/L、（4.05±1.12）mg/L，低于对照组的（12.37±2.65）ng/L、（5.32±1.23）mg/L，差异均有统计学意义（P＜0.05）；观察组患者的足部肉芽组织存在时间及愈合时间分别为（4.97±1.17）、（18.94±3.79）d，均短于对照组的（6.25±1.42）、（23.79±5.05）d，差异有统计学意义（P＜0.05）；观察组治疗总有效率为90.48%，高于对照组的78.57%，差异有统计学意义（P＜0.05）。结论黄芪桂枝汤治疗糖尿病足可以通过降低血糖、改善微炎症状态等机制发挥综合治疗作用，有效促进足部症状的改善。

Abstract：Objective To investigate the effect of Mecobalamin combined with Huangqi Guizhi Decoction in the treatment of diabetic foot and its effect on microinflammation stage.Methods A total of 84 cases of diabetic foot patients treated in our hospital from January 2018 to March 2019 were selected as the research objects.According to the random number table method,they were divided into observation group and control group,42 cases in each group.The patients in the control group received basic treatment and Mecobalamin treatment,while the patients in the observation group received Huangqi Guizhi Decoction on this basis for 4 weeks.The improvement time of foot symptoms clinical efficacy and the changes of facting plasma glecose(FPG),2-hour postprardial blood glucese(2 h PG),glycated hemoglobin(HbA1c),serum tumor necrosis factor-α(TNF-α)and hypersensitive c reactive protein(hs-CRP)before and after treatment were compared between the two groups.Results After treatment,FPG,2 h PG and HbA1c in the observation group were(5.73±1.14),(8.07±1.53)mmol/L,(6.21±0.84)%,respectively,which were lower than those in the control group for(6.23±0.74),(9.27±0.93)mmol/L,(6.92±0.62)%,the differences were of statistically significant(P＜0.05).The TNF-α and hs-CRP were(9.54±2.32)ng/L and(4.05±1.12)mg/L respectively,which were lower than those of the control group for(12.37±2.65)ng/L and(5.32±1.23)mg/L,the differences were of statistically significant(P＜0.05).The time of existence and healing of foot granulation tissue in the observation group were(4.97±1.17)and(18.94±3.79)d,respectively,which were shorter than those in the control group for(6.25±1.42)and(23.79±5.05)d,the differences were of statistically significant(P＜0.05);The total effective rate of the observation group was 90.48%,which was higher than that of the control group(78.57%),the difference was of statistically significant(P＜0.05).Conclusion Huangqi Guizhi Decoction can effectively improve the symptoms of diabetic foot by reducing blood sugar and improving microinflammation.
