丙泊酚联合七氟醚麻醉对小儿腹腔镜疝囊高位结扎术血流动力学及麻醉趋势指数的影响

摘要目的探讨丙泊酚联合七氟醚麻醉对小儿腹腔镜疝囊高位结扎术血流动力学及麻醉趋势指数（NI）的影响。方法选择2015年3月～2016年3月我院收治的99例行腹腔镜疝囊高位结扎术患儿作为研究对象，根据随机抽样法将其分为A组、B组、C组，每组各33例。A组患儿行单一丙泊酚麻醉，B组患儿行单一七氟醚麻醉，C组患儿采用丙泊酚联合七氟醚麻醉。比较三组患儿手术前后的NI、心率（HR）、平均动脉压（MAP）及血氧饱和度（SpO2），并统计三组患儿的不良反应总发生率。结果三组患儿术前的NI比较，差异无统计学意义（P＞0.05）；三组患儿术后的NI均低于术前，差异有统计学意义（P＜0.05）；C组患儿术后的NI高于A组、B组，差异有统计学意义（P＜0.05）；A 组与 B组患儿术后的 NI比较，差异无统计学意义（P＞0.05）。三组患儿术前的 HR、MAP及 SpO2比较，差异无统计学意义（P＞0.05）；三组患儿术后的HR均低于术前，MAP、SpO2均高于术前，差异有统计学意义（P＜0.05）；C组患儿术后的HR高于 A组、B组，MAP、SpO2均低于A组、B组，差异有统计学意义（P＜0.05）；A组与B组患儿术后的HR、MAP及SpO2比较，差异无统计学意义（P＞0.05）。C组患儿的不良反应总发生率低于A组、B组，差异有统计学意义（P＜0.05）；A组与B组患儿的不良反应总发生率比较，差异无统计学意义（P＞0.05）。结论临床上给予小儿腹腔镜疝囊高位结扎术患儿丙泊酚联合七氟醚麻醉可以稳定血流动力学。

Abstract：Objective To investigate the effect of Propofol combined with Sevoflurane anesthesia on hemodynamics and narcotrend index(NI)of laparoscopic high ligation of hernia sac in children.Methods Ninety-nine children who underwent laparoscopic high ligation of hernia sac in our hospital from March 2015 to March 2016 were selected as the research objects.They were divided into group A,group B and group C according to the random sampling method,33 cases in each group.The group A received single Propofol anesthesia,the group B received single Sevoflurane anesthesia,while the group C received Propofol combined with Sevoflurane anesthesia.The NI,heart rate(HR),mean arterial pressure (MAP),and oxygen saturation (SpO2)before and after surgery were compared in the three groups.The total incidence rate of adverse reactions in the three groups of children was counted.Results There was no significant difference in NI among the three groups before surgery (P＞0.05).The NI of the three groups after surgery was lower than that before surgery,and the difference was statistically significant(P＜0.05).The NI of group C after surgery was higher than that of group A and group B,and the differences were statistically significant(P＜0.05).There was no significant difference in the NI between group A and group B after surgery(P＞0.05).There were no significant differences in HR,MAP and SpO2among the three groups before surgery(P＞0.05).The HR after surgery in the three groups was lower than that before surgery,the MAP and SpO2were higher than those before surgery,and the differences were statistically significant (P＜0.05).The HR of group C after surgery was higher than that of group A and group B,the MAP and SpO2were lower than those of group A and group B,and the differences were statistically significant(P＜0.05).There were no significant differences in the HR,MAP and SpO2 between group A and group B after surgery(P＞0.05).The total incidence rate of adverse reactions in group C was lower than that in group A and group B,and the differences were statistically significant(P＜0.05).There was no significant difference in the total incidence rate of adverse reactions between group A and group B(P＞0.05).Conclusion Clinically,the combined anesthesia of Propofol and Sevoflurane can stabilize hemodynamics in children undergoing laparoscopic high ligation of hernia sac.
