右美托咪定联合喷他佐辛对老年患者全麻苏醒期躁动及血流动力学的影响

摘要目的探讨右美托咪定联合喷他佐辛对老年患者全麻苏醒期躁动及血流动力学的影响。方法选取2018年1～12月我院收治的150 例老年患者作为研究对象，按照随机数字表法将其分为D+P 组（75 例）与P 组（75 例）。D+P 组患者采用右美托咪定联合喷他佐辛的方法进行麻醉，P 组患者采用喷他佐辛的方法进行麻醉。比较两组患者手术完成时（T1）、拔气管导管时（T2）、气管导管拔出后5 min（T3）的心率（HR）与平均动脉血压（MAP），观察两组的不良反应发生情况，并记录两组患者的拔除气管导管时间与自主呼吸首次出现时间。结果 D+P 组患者T1～T3时的HR 与MAP 均低于P 组，差异有统计学意义（P＜0.05）；D+P 组患者T2 与T3 的HR、MAP 均高于T1，差异有统计学意义（P＜0.05）；P 组患者T2 与T3 的HR、MAP 均高于T1，差异有统计学意义（P＜0.05）。D+P 组患者的恶心呕吐与躁动发生率均低于P 组，差异有统计学意义（P＜0.05）；D+P 组患者的心动过慢发生率高于P 组，差异有统计学意义（P＜0.05）。D+P 组患者的拔除气管导管时间与自主呼吸首次出现时间均短于P 组，差异有统计学意义（P＜0.05）。结论在老年患者手术期间使用右美托咪定联合喷他佐辛，不良反应的发生率低，血流动力学的稳定性在全麻苏醒期可得到保证，并能缩短拔除气管导管时间与自主呼吸首次出现时间，可以作为老年患者手术的重要麻醉方式，值得在临床中广泛应用。

Abstract：Objective To investigate the effect of Dexmedetomidine combined with Pentazocine on restlessness and hemodynamics during general anesthesia recovery period in elderly patients.Methods A total of 150 elderly patients admitted to our hospital from January to December 2018 were selected as the research objects, and they were divided into group D+P (75 cases) and group P (75 cases) according to the random number table method.Patients in group D+P were anesthetized with Dexmedetomidine combined with Pentazocine, and patients in group P were anesthetized with Pentazocine.The heart rate (HR) and mean arterial blood pressure (MAP) at the completion of surgery (T1), tracheal tube extraction (T2), and 5 minutes after tracheal tube extraction (T3) were compared in the two groups, the incidence of adverse reactions in the two groups was observed,and the time of tracheal tube extraction and the first time of spontaneous respiration in the two groups of patients were recorded.Results The HR and MAP of patients in group D+P at T1-T3 were lower than those in group P, and the differences were statistically significant (P＜0.05).The HR and MAP at T2 and T3 in group D+P were higher than those at T1,and the differences were statistically significant (P＜0.05).The HR and MAP at T2 and T3 in group P were higher than those at T1, and the differences were statistically significant (P＜0.05).The incidence rates of nausea, vomiting and agitation in group D+P were lower than those in group P, and the differences were statistically significant (P＜0.05).The incidence rate of bradycardia in group D+P was higher than that in group P,and the difference was statistically significant (P＜0.05).The time of tracheal tube extraction and the first time of spontaneous respiration in group D+P were shorter than those in group P, and the differences were statistically significant(P＜0.05).Conclusion The use of Dexmedetomidine combined with Pentazocine during surgery in elderly patients has a low incidence of adverse reactions, hemodynamic stability can be guaranteed during the general anesthesia recovery period, and it can shorten the time of tracheal tube extraction and the first time of spontaneous respiration, which can be used as an important anesthesia method for elderly patients, and it is worthy of widespread application in clinic.
