自由体位在产程中的实施及对分娩效果的影响

摘要目的研究自由体位在产程中的应用价值。方法选取2017年4月～2019年1月本院接诊的180例产妇，用电脑随机双盲法分成试验组和对照组，每组各90例。试验组分娩时采取自由体位法，对照组采取传统仰卧位。分析两组的分娩效果，并对各组的产程时间等指标进行比较。结果试验组的自然分娩率为88.89%，高于对照组的66.67%，差异有统计学意义（P＜0.05）。试验组的产后出血量为（209.36±22.37）ml，少于对照组的（223.14±35.18）ml，差异有统计学意义（P＜0.05）。试验组第一、二及三产程时间均短于对照组，差异有统计学意义（P＜0.05）。试验组的总产程时间为（419.18±97.47）min，短于对照组的（531.06±110.28）min，差异有统计学意义（P＜0.05）。试验组对分娩效果总满意度为96.67%，高于对照组的82.22%，差异有统计学意义（P＜0.05）。结论在分娩过程中采取自由体位法，可有效缩短产程时间，提高自然分娩率，改善产妇对分娩效果的满意程度，建议临床推广应用。

Abstract：Objective To study the application value of free position during labor process.Methods A total of 180 parturient women who received treatment in our hospital from April 2017 to January 2019 were selected and computer randomized double blind method was used to divide the experimental group and the control group,90 cases in each group.The experimental group was adopted the free position method during childbirth,and the control group was adopted the traditional supine position.The parturient effect of the two groups was analyzed,and the parturient duration and other indicators of two groups were compared.Results The rate of spontaneous delivery was 88.89%,higher than that of control group (66.67%),the difference was statistically significant(P＜0.05）.The postpartum blood loss in the experimental group was(209.36±22.37)ml,less than(223.14±35.18)ml in the control group,and the difference was statistically significant(P＜0.05）.The first,second and third labor stages of the experimental group were shorter than those of the control group,and the difference was statistically significant(P＜0.05）.The total duration of labor in the experimental group was(419.18±97.47)min,shorter than(531.06±110.28)min in the control group,and the difference was statistically significant(P<0.05）.The total satisfaction of the experimental group with the delivery effect was 96.67%,higher than that of the control group (82.22%),and the difference was statistically significant(P＜0.05）.Conclusion The method of free posture can effectively shorten the time of labor,increase the rate of natural delivery,improve the degree of satisfaction of the parturient with the effect of delivery,and suggest clinical popularization.
