宫颈球囊与催产素联合应用于妊娠晚期引产的临床疗效

摘要目的探讨宫颈球囊与催产素联合应用于妊娠晚期引产的临床疗效。方法选取2016年1月1日～2017年12月31日我收治的100例妊娠晚期引产孕妇作为研究对象，按照单月、双月并结合孕妇自己的意愿将其分为对照组（n=50）和研究组（n=50）。对照组孕妇行小剂量催产素静脉滴注的引产方式，研究组孕妇在对照组的基础上采用宫颈球囊引产。比较两组孕妇促宫颈成熟总有效率、促宫颈成熟前后宫颈Bishop评分、引产相关情况及不良反应总发生率。结果研究组孕妇的促宫颈成熟有效率高于对照组，差异有统计学意义（P<0.05）。两组孕妇促宫颈成熟前的Bishop评分比较，差异无统计学意义（P>0.05）；两组孕妇促宫颈成熟后的Bishop评分均高于促宫颈成熟前，差异有统计学意义（P<0.05）；研究组孕妇促宫颈成熟后的宫颈Bishop评分高于对照组，差异有统计学意义（P<0.05）。研究组孕妇的用药次数、产后出血发生率、剖宫产率低于对照组，总产程时间短于对照组，阴道分娩发生率高于对照组，差异有统计学意义（P<0.05）。两组孕妇的不良反应总发生率比较，差异无统计学意义（P>0.05）。结论妊娠晚期引产应用宫颈球囊与催产素联合治疗的效果确切，且不良反应发生率低，具有较高的治疗安全性，值得推广应用。

Abstract：Objective To investigate the clinical effect of cervical balloon combined with Oxytocin in the induction of labor in the late pregnancy.Methods One hundred pregnant women with induced labor in the late pregnancy who were admitted to our hospital from January 1st,2016 to December 31st,2017 were selected as subjects.They were divided into control group (n=50)and study group (n=50)according to single month and double month combined with the patients′own wishes.The control group received intravenous infusion of small dose of Oxytocin,and the study group received cervical balloon on the basis of the control group.The total effective rate of cervical maturation,the cervical Bishop scores before and after cervical maturation,the induction of labor and the total incidence of adverse reactions were compared between two groups.Results The effective rate of cervical maturation in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the cervical Bishop scores before cervical maturation between two groups(P>0.05).The cervical Bishop scores after cervical maturation in two groups were higher than those before cervical maturation,and the differences were statistically significant(P<0.05).The cervical Bishop scores after cervical maturation in the study group were higher than those before cervical maturation,and the difference was statistically significant(P<0.05).The frequency of medication,the incidence of postpartum hemorrhage,and the rate of cesarean section in the study group were lower than those in the control group,the total labor time was shorter than that in the control group,the incidence of vaginal delivery was higher than that in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between two groups(P>0.05).Conclusion The combination use of cervical balloon and Oxytocin is effective in the induction of labor in the late pregnancy,and the incidence of adverse reactions is low,which has high safety in treatment.It is worthy of promotion and application.
