胸腔镜辅助食管癌根治术与开胸食管癌根治术治疗老年食管癌的效果及对术后恢复的影响对比

摘要目的比较胸腔镜辅助食管癌根治术与开胸食管癌根治术治疗老年食管癌的效果及对术后恢复的影响。方法选取2015年3月～2016年2月我院收治的85例食管癌患者作为研究对象，按照病床单双号分为对照组（42例）和观察组（43例），其中对照组行开胸根治术治疗，观察组行胸腔镜辅助根治术治疗，门诊随访2年统计术后相关并发症及2年内死亡、复发、转移发生情况，比较不同术式患者术后恢复、手术、生活质量差异。结果两组复发、死亡、转移率比较，差异无统计学意义（P>0.05）；观察组并发症发生率（9.3%）低于对照组（21.4%），差异有统计学意义（P<0.05）；观察组出血量少于对照组，住院及术后拔管时间短于对照组，差异有统计学意义（P<0.05）；两组手术耗时、淋巴结清扫数目比较，差异无统计学意义（P>0.05）；两组治疗后生活质量评分均高于治疗前，且观察组生活质量评分高于对照组，差异有统计学意义（P<0.05）。结论食管癌患者临床以胸腔镜辅助食管癌根治术治疗，患者预后效果较好，术后恢复快、并发症少，生活质量显著提高。

Abstract：Objective To compare the effect of thoracoscope assisted radical resection of esophageal carcinoma and open radical resection for esophageal cancer in elderly patients with esophageal cancer and the influence on postoperative recovery.Methods Eighty-five patients with esophageal cancer who were admitted to our hospital from March 2015 to February 2016 were selected as study subjects.They were divided into control group (n=42)and observation group (n=43)according to the odd/even number of the beds.In the control group,open radical resection was used,while in the observation group,thoracoscope assisted radical resection was adopted.The postoperative related complications,and deaths,recurrences,and metastases occurred within 2 years after surgery by outpatient follow-up were calculated.The differences in postoperative recovery,surgery and quality of life were compared between two different surgical procedures.Results There was no significant difference in recurrence,death and metastasis rate between the two groups (P>0.05).The incidence of complications in the observation group (9.3%)was lower than that in the control group(21.4%),the difference was statistically significant(P<0.05).The amount of bleeding in the observation group was less than that in the control group.The hospital stay and time of postoperative extubation were shorter in the observation group than in the control group,the difference was statistically significant(P<0.05).There was no significant difference in surgical time or amount of lymph node dissection between the two groups(P>0.05).The quality of life scores after treatment in the two groups were higher than that before treatment,and the quality of life score in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion For esophageal cancer patients,thoracoscope assisted radical resection obtains a great prognosis effect with fast postoperative recovery,few complications and significant improvement of quality of life.
