开放与闭合复位延迟治疗儿童股骨颈骨折的临床效果比较

摘要目的探讨对儿童股骨颈骨折实施开放、闭合复位延迟治疗的效果，为临床提供指导。方法选取2007年8月～2017年8月我院收治的68例股骨颈骨折患儿为观察对象，按治疗方法的不同将其分开放组（38例）、闭合组（30例）。开放组应用开放复位延迟治疗，闭合组应用闭合复位延迟治疗。均随访半年，比较两组的疗效、髋关节功能评分、骨折愈合时间及并发症发生率。结果开放组的优良率为81.58%，高于闭合组（50.00%），差异有统计学意义（P＜0.05）；两组的骨折愈合时间比较，差异无统计学意义（P＞0.05），但开放组治疗后半年的髋关节功能评分[（87.65±3.20）分],高于闭合组[（78.32±4.25）分]，差异有统计学意义（P＞0.05）；开放组的并发症发生率为 5.26%，低于闭合组（26.67%），差异有统计学意义（P＜0.05）。结论对股骨颈骨折患儿实施开放复位延迟治疗的效果较闭合复位延迟治疗更佳，更有助于改善患儿髋关节功能，且并发症更少。

Abstract：Objective To explore the effect of open and closed reduction delayed treatment in children with femoral neck fracture.Methods From August 2007 to August 2017,68 children with femoral neck fracture in our hospital were selected as observation objects,they were divided into two groups according to different treatment method,38 cases in closed group,and 30 cases in open group.Open group was treated with open reduction delayed treatment,closed group was treated with closed reduction delay.All patients were followed up for half a year.The curative effect,hip function score,fracture healing time and incidence of complications were compared between open group and closed group.Results The excellent and good rate of open group(81.58%)was higher than that of closed group(50.00%)(P＜0.05）.Fracture healing time of two groups there was no significant difference(P＞0.05）,but the score of hip joint function in the open group of(87.65±3.20)score was significantly higher than that in the closed group of(78.32±4.25)score half a year after treatment,and the difference was statistically significant.The incidence of complications in the open group(5.26%)was lower than that in the closed group,(26.67%),with a statistical difference(P＜0.05）.Conclusion The effect of open reduction delay therapy in children with femoral neck fracture is better than that in closed reduction delay treatment.It is more helpful to improve hip function and has fewer complications in children with femoral neck fracture.
