血清脂蛋白a、胱抑素C和尿酸的联合检测在高血压并发脑梗死患者中的诊断价值

摘要目的探讨联合检测血清脂蛋白a[Lp（a）]、胱抑素C（Cys-C）及尿酸（UA）在高血压并发脑梗死中的诊断价值。方法选择2014年4月~2015年10月我院64例高血压并发脑梗死患者作为观察组，选择我院同期72例单纯高血压患者作为对照组，分别采用胶乳增强免疫透射比浊法和重组尿酸氧化法测定两组患者的Lp（a）、Cys-C 及UA水平，比较两组Lp（a）、Cys-C及UA的表达水平，并评估Lp（a）、Cys-C及UA联合检测在高血压并发脑梗死中的诊断价值。结果观察组Lp（a）、Cys-C及UA分别为（379.64±60.28）mg/L、（1.03±0.24）mg/L及（515.60± 64.42）μmol/L，均显著高于对照组的（302.35±47.91）mg/L、（0.86±0.07）mg/L及（427.83±47.65）μmol/L，差异均有统计学意义（P＜0.05）。Lp（a）、Cys-C及UA三项联合检测的AUC、特异度和敏感度分别为0.923、86.72%和97.24%，均高于Cys-C（0.615、70.32%、84.77%）和Cys-C联合Lp（a）（0.843、76.84%、88.09%）、Cys-C联合UA（0.778、73.21%、87.35%）。结论高血压并发脑梗死患者Lp（a）、Cys-C及UA存在高表达现象，三项联合检测可有效提高高血压并发脑梗死诊断准确率，对高血压并发脑梗死患者的防治及改善预后效果具有较高的临床指导意义。

Abstract：Objective To investigate the effectiveness of combination of detection of serum lipoprotein a[Lp（a）],cystatin C (Cys-C)and uric acid (UA)in the diagnosis of hypertension patients complicated with cerebral infarction.Methods Altogether 64 patients with hypertension complicated with cerebral infarction who were treated in our hospital from April 2014 to October 2015 were selected as observation group;while,72 patients with hypertension who were treated in the hospital at the same period were selected as control group.Lp（a）,Cys-C and UA of the two groups were measured by using latex-enhanced immunoturbidimetric assay and recombinant uric acid oxidation;and the expression levels of Lp（a）,Cys-C and UA were compared between the two groups;the value of combination of detection of Lp（a）,Cys-C and UA in the diagnosis of hypertension patients complicated with cerebral infarction was evaluated.Results Lp（a）, Cys-C and UA of observation group were (379.64±60.28)mg/L,(1.03±0.24)mg/L and (515.60±64.42)μmol/L,respectively,significantly higher than control group[(302.35±47.91)mg/L,(0.86±0.07)mg/L and(427.83±47.65)μmol/L],with statistically significant difference(P<0.05).AUC,sensitivity and specificity of combination of detection of Lp（a）,Cys-C and UA were 0.923,86.72%and 97.24%,higher than those of single Cys-C(0.615,70.32%,84.77%),combination of Cys-C and Lp（a）(0.843,76.84%,88.09%)or UA(0.778,73.21%,87.35%).Conclusion High expression of Lp（a）,Cys-C and UA is seen in patients with hypertension complicated with cerebral infarction.Combination of detection of Lp（a）,Cys-C and UA can effectively improve the accurate rate in diagnosis of hypertension patients complicated with cerebral infarction,which has high clinical significance on the prevention and treatment and improved prognosis in patients with hypertension complicated with cerebral infarction.
