Clinical effect analysis of deep anterior lamellar keratoplasty in the treatment of herpes simplex keratitis

Abstract:ObjectiveTo investigate the clinical effect of deep anterior lamellar keratoplasty(DALK)in the treatment of herpes simplex keratitis.MethodsA total of 70 patients with herpes simplex keratitis treated in our department from January 2014 to January 2016 were selected and randomly divided into the control group and the observation group,35 cases in each group.The control group was treated with penetrating keratoplasty(PKP),the observation group was treated with DALK.The surgical efficacy,visual acuity,graft rejection and survival,the incidence rate of complications and recurrence rate in the two groups were compared.ResultsThe total effective rate of the observation group was 97.14%,which was significantly higher than 82.86%of the control group,the difference was statistically significant(P<0.05).There was no significant difference between the observation group and the control group in the equivalent spherical lens value and astigmatism(P>0.05);The survival rate of skin grafts in observation group was 97.14%,and the rejection rate was 2.86%;the skin graft survival rate of the control group was 80.00%,and the rejection rate was 20.00%,the difference was statistically significant(P<0.05).The incidence rate of complication such as secondary glaucoma,localized anterior synechia of iris,secondary cataract,posterior elastic layer micro perforation and posterior elastic layer wrinkle were significantly lower in the observation group than in the control group,the difference was statistically significant(P<0.05).The recurrence rate of epithelial and stromal viruses in the observation group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).There was no recurrence of endothelial virus in the two groups.ConclusionDALK is effective in the treatment of herpes simplex keratitis,and it has high survival rate,low rejection rate,no obvious effect on postoperative visual acuity and low incidence of complications.It has positive clinical significance.
