穴位注射联合耳穴贴压对胃癌患者术后胃肠功能和免疫功能的影响

摘要目的观察穴位注射联合耳穴贴压对胃癌患者术后胃肠功能和免疫功能的影响。方法将2013年1月～ 2015年12月到四川省自贡市中医医院就诊的符合标准的86例患者随机分为对照组和治疗组。其中对照组45例，采用西医常规术后处理；治疗组41例，在对照组基础上加用中药参芪扶正注射液足三里穴位注射和双侧耳穴穴位压豆治疗。观察患者术后肠鸣音恢复时间、首次排气时间和排便时间，收集患者治疗前后鼻胃管内胃液量，测定血清胃泌素浓度和外周血T淋巴细胞亚群水平。结果治疗组患者肠鸣音恢复时间、首次排气时间和排便时间均短于对照组，差异均有统计学意义（P＜0.05）。两组患者治疗后胃液量下降，胃泌素上升，与本组治疗前比较，胃液量差异有统计学意义（P＜0.05），胃泌素差异无统计学意义（P＞0.05）。治疗组治疗后的胃液量少于对照组，差异有统计学意义（P＜0.05），胃泌素高于对照组，差异无统计学意义（P＞0.05）。两组患者CD3、CD4和CD8淋巴细胞计数均升高，CD4/CD8值降低，与本组治疗前比较，差异有统计学意义（P＜0.05）；组间比较，治疗组患者优于对照组，差异有统计学意义（P＜0.05）。结论胃癌术后早期应用参芪扶正注射液足三里穴位注射联合耳穴贴压能促进患者胃肠功能恢复，提高免疫功能，疗效优于单纯西医治疗。

Abstract：Objective To observe the influence of acupoint injection combined with auricular plaster therapy on gastrointestinal function and immune function in postoperative patients with gastric cancer.Methods 86 patients with gastric cancer meeting the standard treated by Traditional Chinese Medicine Hospital of Zigong City in Sichuan Province from January 2013 to December 2015 were randomly divided into control group and treatment group.In control group,45 cases were given the routine postoperative treatment of western medicine,while in treatment group,41 cases was treated by Shenqi Fuzheng Injection of traditional Chinese medicine in Zusanli acupoint to inject and bilateral ear acupoint pressing bean on the basis of control group.Recovery time of bowel sound after operation,first exhaust and defecation time in patients was observed respectively;the gastric juice volume in nasogastric tube before and after treatment in patients was collected,and serum gastrin concentration and level of T lymphocyte subgroup was determined respectively. Results In patients of treatment group,the recovery time of bowel sound,the first exhaust and defecation time was shorter than that of control group,the difference was statistically significant(P<0.05).Gastric juice volume was decreased and gastrin was rised after treatment in two groups,and compared with the group before treatment,the difference of gastric juice volume was statistically significant(P<0.05),the difference of gastrin was not statistically significant(P>0.05).After treatment,gastric juice volume in treatment group was less than that of control group,the difference was statisti-cally significant(P<0.05),and the gastrin was higher than that of control group,the difference was not statistically significant(P>0.05).Lymphocyte count of CD3,CD4 and CD8 was increased and CD4/CD8 value was decreased in two groups, and compared with the group before treatment,the difference was statistically significant(P<0.05);comparison between groups:the patients in treatment group were better than those of control group,the difference was statistically significant (P<0.05).Conclusion Early application of Shenqi Fuzheng Injection with auricular plaster therapy after operation can promote the recovery of gastrointestinal function and improve immune function in patients and the curative effect is better than that of treatment of western medicine.
