Abstract：Objective To study the related factors influencing clinical judgment in treatment of traumatic splenic rupture.Methods 139 cases of traumatic spleen rupture treated in our hospital from March 2012 to September 2015 were selected，and the factors influencing the treatment plan were analyzed by Logistic regression analysis.Results Causes of injuries，pulse，amount of bleeding，abdominal puncture，injury classification and whether complicated with organ injury were closely related with therapeutic scheme；while themulti factor Logistic regression analysis confirmed that the difference of gender，hospitalization time and treatment scheme was not statistically significant.55 cases in conservative treatment patientswere gradeⅠ，gradeⅡpatients，accounting for 39.6% （55/139），84 cases surgical patients accounted for 60.4% （84/139），spleen excision rate was 47.6%in patientswith gradeⅠandⅡ，100.0%in patientswith gradeⅢandⅣ；Success rate of surgical treatmentwas 90.8%，success rate of conservative treatment with 89.1%，the difference was not statistically significant（P＞0.05），success rate of conservative treatment for traumatic splenic rupture was higher，close to success rate of surgical treatment.Conclusion The whole spleen resection ismain therapeutic method of traumatic splenic rupture，and operation protecting spleen or nonoperative treatment is a safe and effectivemethod of treatment according to causes of injuries，pulse，amount of bleeding，abdominal puncture，injury classification，whether complicated with organ injury and other factors.
