人文关怀护理对人工髋关节置换术后谵妄患者临床症状的影响

摘要目的探讨人文关怀护理对人工髋关节置换术（THA）后谵妄患者临床症状的影响。方法选取2018年1月—2020年10月福建医科大学附属第一医院收治的100 例THA 后谵妄患者作为研究对象，按照随机数字表法分为实验组（50 例）与对照组（50 例）。对照组采用常规护理方法，实验组采用人文关怀护理方法。比较两组的谵妄评分量表（CAM）评分、谵妄持续时间、住院时间。结果实验组的术后24、48、72 h CAM 评分、出院前评分低于对照组，差异均有统计学意义（P<0.05）；实验组的术后48、72 h、出院前及对照组术后72 h、出院前CAM 评分低于术后24 h，差异有统计学意义（P<0.05）。实验组的谵妄持续时间、住院时间短于对照组，差异均有统计学意义（P<0.05）。实验组的护理满意度评分高于对照组，差异有统计学意义（P<0.05）。结论 THA 后谵妄患者接受人文关怀护理干预，能更好对患者术后谵妄程度控制，缩短谵妄持续时间及住院时间，且患者的满意度较高，有临床应用意义。

Abstract：Objective To explore the effect of humanistic care on the clinical symptoms of patients with confusion after artificial hip replacement(THA).Methods A total of 100 patients with delirium after THA who were admitted to the First Affiliated Hospital of Fujian Medical University from January 2018 to October 2020 were selected as the research subjects.According to the random number table method,they were divided into the experimental group(50 cases)and the control group(50 cases).The control group was treated with conventional nursing and the experimental group was treated with humanistic care nursing.The confusion assessment method(CAM)score,duration of confusion,and hospital stay between the two groups were compared.Results The 24,48,72 h,and pre-discharge CAM scores of the experimental group were lower than those of the control group after surgery,the CAM scores of the experimental group at 48,72 h,before discharge,and the control group at 72 h and before discharge were lower than 24 h after operation,the differences were statistically significant(P<0.05).The duration of confusion and hospital stay in the experimental group were shorter than those in the control group,the nursing satisfaction score was higher than that of the control group,the differences were statistically significant(P<0.05).Conclusion Patients with confusion after THA receiving humanistic care intervention can greatly control the degree of postoperative confusion,shorten the duration of confusion and hospital stay,and the patients′ satisfaction is high,which is of great application significance.
