Timing of operation and its influence on prognosis of cholecystolithiasis complicated with mild to moderate pancreatitis

Abstract:Objective To explore the influence on prognosis of different operation time on cholecystolithiasis complicated with mild to moderate pancreatitis. Methods A total of 60 patients with cholecystolithiasis complicated with mild to moderate pancreatitis admitted to Nantong Hospital of Traditional Chinese Medicine from January 2017 to August 2020 were selected as the research objects, and divided into the control group and the observation group according to the random number table method, with 30 cases in each group. The control group received surgical treatment from 72 hours to 14 days after onset, and the observation group received surgical treatment within 72 hours. The serum levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), amylase (AMY), surgical indicators (including the operation time, interoperative bleeding volume, postoperative feeding time and postoperative hospitalization time), total complication rate and quality of life score (including the MOS item short from health survey[SF-36] and visual analog scale [VAS] scores). Results After the surgery, the levels of AST, ALT, ALP and AMY in two groups were lower than those in this group before the surgery, and the levels of AST, ALT, ALP and AMY in the observation group were lower than those in the control group, the differences were statistically significant (P<0.05).There were no significant differences in the operation time, interoperative bleeding volume, postoperative feeding time between the two groups (P>0.05); and the postoperative hospitalization time of the observation group was shorter than that of the control group, the difference was statistically significant (P<0.05). The total complication rate of the observed group was lower than that of the control group,the difference was statistically significant (P<0.05). After the surgery, the SF-36 score of the two groups were higher than those of this group before the surgery, and the SF-36 score of the observation group was higher than that of the control group, the differences were statistically significant (P<0.05). After the surgery, the VAS score of the two groups were lower than those of this group before the surgery, and the VAS score of the observation group was lower than that of the control group, the differences were statistically significant(P<0.05). Conclusion The clinial effect of patients with gallstone complicated with mild to moderate pancreatitis were received surgical treatment within 72 hours is better, and the effect is good. It could effectively shorten the time of hospitalization, reduce infection, reduce the occurrence of complications, and could be used in clinical practice.
