加味承气汤对重症急性胰腺炎患者凝血功能的影响

摘要目的探讨加味承气汤对重症急性胰腺炎患者凝血功能的影响。方法选取2018年4月—2020年6月景德镇市第二人民医院收治的96 例重症急性胰腺炎患者，采用随机数字表法分为实验组与对照组，每组各48 例。实验组接受常规综合治疗联合加味承气汤治疗，对照组接受常规综合治疗。比较两组患者治疗前后的凝血功能[纤维蛋白原（FBG）、活化部分凝血活酶时间（APTT）、凝血酶原时间（PT）、血小板计数（PLT）、凝血酶时间（TT）]及生化指标（血清钙浓度、血淀粉酶）。结果治疗后，实验组的FBG 水平低于对照组，APTT、PT、TT 短于对照组，PLT 水平高于对照组，差异有统计学意义（P＜0.05）；治疗后，实验组的血清钙浓度高于对照组，血淀粉酶水平低于对照组，差异有统计学意义（P＜0.05）。结论在重症急性胰腺炎患者中采取加味承气汤治疗能够帮助患者恢复凝血功能，临床治疗效果显著，值得推广应用。

Abstract：Objective To explore the influence of Jiawei Chengqi Decoction on coagulation function in patients with severe acute pancreatitis.Methods From April 2018 to June 2020,96 patients with severe acute pancreatitis from the Second People′s Hospital of Jingdezhen City were selected,and they were divided into the experimental group and the control group by random number table method.The experimental group received conventional comprehensive treatment combined with Jiawei Chengqi Decoction treatment,the control group received conventional comprehensive treatment.The coagulation function(fibrinogen[FBG],activated partial thromboplastin time[APTT],prothrombin time[PT],platelet count[PLT],and thrombin time[TT])and biochemical indicators(serum calcium concentration,serum amylase)of the two groups were compared.Results After treatment,the level of FBG of the experimental group was lower than that of the control group,APTT,PT,TT of the experimental group were shorter than those of the control group,and the level of PLT indicators of the experimental group were higher than that of the control group,the differences were statistically significant(P＜0.05).After treatment,the serum calcium concentration of the experimental group was higher than that of the control group,and the blood amylase of the experimental group was lower than that of the control group,and the differences were statistically significant(P＜0.05).Conclusion The treatment of Jiawei Chengqi Decoction in patients with severe acute pancreatitis can help patients restore blood coagulation function and improve the quality of life of patients,the clinical treatment effect is significant,and it is worthy of promotion and application.
