窄带成像内镜在消化道早癌及癌前病变筛查中的诊断价值

摘要目的探讨窄带成像内镜在消化道早癌及癌前病变筛查中的诊断价值。方法选取2018年8月～2020年5月阳江市人民医院收治的100例经幽门螺杆菌相关检测、胃功能三项检查、大便隐血试验确诊的消化道早癌及癌前病变患者作为研究对象。采用简单随机化法将患者分为观察组和对照组，每组各50例。观察组患者接受窄带成像内镜诊断筛查；对照组患者采取传统内镜观察诊断。比较两组患者的消化道早癌及癌前病变检出率、癌症类型，比较两组患者的检查成像病变轮廓评分、微血管形态评分及胃小凹形态评分。结果观察组的消化道早癌及癌前病变检出率（98.0%）高于对照组（78.0%），差异有统计学意义（P＜0.05）；两组的癌症类型检查结果分布情况比较，差异有统计学意义（P＜0.05）；观察组患者的检查成像病变轮廓评分、微血管形态评分及胃小凹形态评分均高于对照组，差异有统计学意义（P＜0.05）。结论消化道早癌及癌前病变的筛查诊断中，使用窄带成像内镜的检出率高，并且能够对患者的癌症类型有较准确的描述，成像质量也较高，能准确观察病灶组织，临床推广价值高。

Abstract：Objective To explore the diagnostic value of narrow-band imaging endoscopy in screening early gastrointestinal cancer and precancerous lesions of the digestive tract.Methods A total of 100 patients admitted to Yangjiang People′s Hospital from August 2018 to May 2020 who were confirmed with helicobacter pylori related tests,gastric function tests,and stool occult blood tests were selected as the research objects.The patients were divided into observation group and control group by simple randomization,with 50 cases in each group.Observation group received endoscopic diagnosis screening with narrow-band imaging endoscopy;the control group was diagnosed by traditional endoscopic observation.The detection rates of early gastrointestinal cancer and precancerous lesions in the digestive tract and the types of cancers were compared between the two groups.The lesion contour score,microvascular morphological score and gastric pit morphological score of examine imaging were compared between the two groups.Results The detection rate of early gastrointestinal cancer and precancerous lesions of digestive tract in observation group(98.0%)was higher than that in control group(78.0%),and the difference was statistically significant(P＜0.05).There was statistically significant difference between the two groups in the results of cancer type examination(P＜0.05).The lesion contour score,microvascular morphological score and gastric pit morphological score of examine imaging in observation group were higher than those in control group,the differences were statistically significant(P＜0.05).Conclusion In the screening and diagnosis of early gastrointestinal cancer and precancerous lesions,narrowband imaging endoscopy has a high detection rate,and can accurately describe the type of cancer in patients.The imaging quality is also high,and the lesion tissue can be accurately observed,which is of high clinical promotion value.
