单孔充气法腔镜保乳手术治疗早期乳腺癌的临床效果

摘要目的观察单孔充气法腔镜保乳手术治疗在早期乳腺癌患者中的临床效果。方法选取2017年11月～2019年11月北京市房山区良乡医院乳腺外科收治的86例早期乳腺癌患者作为研究对象，依据随机数字表法将其分为观察组及对照组，每组各43例。观察组患者行单孔充气法腔镜保乳手术，对照组患者行开放保乳手术。比较两组患者的围术期指标、术后常见并发症、乳房外观满意度情况。结果 86例患者均成功完成保乳术，成功率为100.00%。观察组患者的手术时间长于对照组，术中出血量少于对照组，切口总长度短于对照组，差异有统计学意义（P＜0.05）；两组患者的术后引流总量、引流时间及住院时间比较，差异均无统计学意义（P＞0.05）。观察组患者的术后并发症总发生率低于对照组，差异有统计学意义（P＜0.05）。观察组患者的乳房外观满意度高于对照组，差异有统计学意义（P＜0.05）。结论单孔充气法腔镜保乳手术治疗早期乳腺癌的临床效果与开放保乳手术相比，具有创伤小、并发症少的优势，能获得更优的美容效果。

Abstract：Objective To investigate the clinical effect of single-port laparoscopic breast-conserving surgery on patients with early breast cancer.Methods A total of 86 patients with early breast cancer admitted to the Department of Breast Surgery,Liangxiang Hospital in Fangshan District,Beijing from November 2017 to November 2019 were selected as the research objects.According to the random number table method,they were divided into observation group and control group,with 43 cases in each group.The observation group was treated by single-port laparoscopic breast-conserving surgery,and the control group was treated by open breast-conserving surgery.The perioperative indexes,common postoperative complications and breast appearance satisfaction were compared between the two groups.Results Breast conserving surgery was successfully completed in 86 patients with a success rate of 100.00%.The operation time of observation group was longer than that in the control group,the amount of intraoperative blood loss was less than that in the control group,and the total length of incision was shorter than that in the control group,and the differences were statistically significant(P＜0.05).There were no statistical significances in the total amount of postoperative drainage,drainage time and hospital stay between the two groups(P＞0.05).The total incidence of postoperative complications in observation group was lower than that in the control group,and the difference was statistically significant(P＜0.05).The breast appearance satisfaction of observation group was higher than that of control group,and the difference was statistically significant(P＜0.05).Conclusion Compared with open breast-conserving surgery,the single-port laparoscopic breast-conserving surgery has such advantages as smaller wound,less complications,and better cosmetic outcomes in treating patients with early breast cancer.
