活检术前应用皮质醇对原发性中枢神经系统淋巴瘤病理诊断的影响

摘要目的探讨活检术前应用皮质醇对原发性中枢神经系统淋巴瘤病理诊断的影响。方法回顾性分析2017年8月～2019年10月牡丹江医学院附属红旗医院诊治的180 例原发性中枢神经系统淋巴瘤患者的临床资料，根据是否应用皮质醇分为术前应用皮质醇组（91 例）与术前未应用皮质醇组（89 例），比较两组的病理诊断结果。结果术前应用皮质醇组中，有5 例（5.49%）未得到阳性结果，再次行活检术后确诊原发性中枢神经系统淋巴瘤；术前未应用皮质醇组有3 例（3.37%）未得到阳性结果，再次行活检术检验后确诊原发性中枢神经系统淋巴瘤；两组的病例检测结果比较，差异无统计学意义（P＞0.05）。结论对于大部分原发性中枢神经淋巴瘤患者而言，在活检术前应用皮质醇并不会对病理诊断产生明显影响。

Abstract：Objective To investigate the effect of cortisol administration before biopsy on the pathologic diagnosis of primary central nervous system lymphoma.Methods The clinical data of 180 patients with primary central nervous system lymphoma treated in Hongqi Hospital Affiliated to Mudanjiang Medical College from August 2017 to October 2019 were retrospectively analyzed.According to whether or not cortisol was used, the patients were divided into the preoperative cortisol group (91 cases) and the non-preoperative cortisol group (89 cases), and the pathological diagnosis results of the two groups were compared.Results In the preoperative corticosterol group, 5 cases (5.49%) did not get positive results, and primary central nervous system lymphoma was confirmed after re-biopsy.In the preoperative non cortisol-treated group, 3 cases (3.37%) did not get positive results, and primary central nervous system lymphoma was confirmed after re-biopsy.There was no significant difference in case detection results between the two groups (P>0.05).Conclusion For most patients with primary central nervous system lymphoma, the application of cortisol before biopsy has no significant impact on the final pathological diagnosis.
