Abstract:Objective To use Meta-analysis to compare the effect of perioperative platelet transfusion and non-platelet transfusion on the prognosis of adult cardiac surgery patients,so as to provid clinical evidence of evidence-based medicine.Methods Retrospective studies on the effect of perioperative platelet transfusion on prognosis in adult patients undergoing cardiac surgery were searched from China National Knowledge Infrastructure,Wanfang,China Biomedical Literature Database,EMbase and PubMed databases.The retrieval time was from January 2000 to July 2020.Review Manager 5.4 software was used for Meta-analysis.Results Eight retrospective studies involving 36 654 patients were included in this study.Of all patients enrolled in the study,17%(6232/36 654)received perioperative platelet transfusion and 83%(30 422/36 654)did not receive platelet transfusion.The results of Meta-analysis showed that there were no significant differences in 30 d postoperative death,number of reoperations,postoperative blood transfusion,operative time and ICU retention time between the platelet transfusion group and the non-transfusion group during the perioperative period of heart surgery(P>0.05).However,the postoperative infection in the platelet infusion group was higher than that in the non-platelet infusion group(RR=0.03,95%CI: 0.00-0.05,P=0.04),and the number of patients with myocardial infarction was also higher than that in the non-platelet infusion group(RR=1.78,95%CI: 1.18-2.70,P=0.008),with statistical significances.Conclusions Perioperative platelet transfusion and non-transfusion of platelets in adult cardiac surgery patients had no significant effect on postoperative complications,mortality,operative time,and ICU retention time,but platelet transfusion can increase the incidence of postoperative infection and myocardial infarction.
