肝内胆管囊腺瘤1例

摘要肝内胆管囊腺瘤（IBC）属于良性肿瘤，发病率低，病因尚未明确，临床症状缺乏特异性，易与肝囊肿、肝脓肿、肝包虫病等肝脏疾病相混淆。该肿瘤进展缓慢，但具有恶变风险，治疗原则是早期手术完整切除肿瘤。现回顾性报道IBC 一例。该病例腹部超声检查见肿瘤呈囊性，肝内无回声区，内含多发分隔的液性暗区及光带漂浮。病理检查见囊壁由纤维组织构成，局部见卵巢样间质，局灶被覆单层立方/柱状上皮，符合胆管黏液性囊腺瘤。报道该病例影像和诊疗分析，旨在提升对IBC的认识，减少误诊的发生。

Abstract：Intrahepatic biliary cystadenoma (IBC) is a benign tumor with low incidence, unknown etiology and lack of specificity in clinical symptoms.It is easy to be confused with liver diseases such as liver cyst, liver abscess and liver echinococcosis.IBC progresses slowly, but is at risk of malignant transformation.The principle of treatment is early surgical removal of the tumor completely.A case of IBC was reported retrospectively.Ultrasound examination of the abdomen in this case revealed cystic tumor, anechoic areas in the liver, contains multiple separate liquid dark areas and light bands floating.Pathological examination revealed that the cyst wall was composed of fibrotic tissue, with local ovarioid stroma and a single layer of cubic/columnar epithelium.It was consistent with a mucinous cystadenoma of the bile duct.To report the image and diagnosis and treatment analysis of this case, aim to improve the understanding of IBC and reduce the incidence of misdiagnosis.
