全身麻醉复合硬膜外阻滞麻醉在腹腔镜手术治疗老年直肠癌中的临床效果

摘要目的探讨全身麻醉复合硬膜外阻滞麻醉在腹腔镜手术治疗老年直肠癌中的应用效果。方法选取2017年2月～2019年6月我院收治的108例老年直肠癌患者的临床资料进行回顾性分析，根据不同的治疗方式将患者分为对照组（54例）与研究组（54例）。对照组患者采用全身麻醉，研究组患者采用全身麻醉复合硬膜外阻滞麻醉。比较两组患者的麻醉效果及心率（HR）、收缩压（SBP）、舒张压（DBP）情况。结果两组患者麻醉前的HR、SBP、DBP 比较，差异无统计学意义（P＞0.05）；研究组患者插管时、拔管时的HR、SBP 均低于对照组，差异有统计学意义（P＜0.05）；两组患者拔管后的HR、SBP 比较，差异无统计学意义（P＞0.05）。两组患者气腹后20 min 的SBP、DBP均高于麻醉前，差异有统计学意义（P＜0.05）；两组患者放气后20 min 的SBP、DBP 与麻醉前比较，差异无统计学意义（P＞0.05）；研究组患者气腹后20 min 的SBP、DBP 均低于对照组，差异有统计学意义（P＜0.05）。结论直肠癌根治手术中采用全身麻醉复合硬膜外阻滞麻醉的临床效果良好，安全性较高，值得在临床上推广应用。

Abstract：Objective To explore the application effect of general anesthesia combined with epidural anesthesia in laparoscopic surgery for elderly rectal cancer.Methods The clinical data of 108 elderly patients with rectal cancer treated in our hospital from February 2017 to June 2019 were analyzed retrospectively.According to different treatment methods, the patients were divided into control group (54 cases) and study group (54 cases).Patients in the control group received general anesthesia, and in the study group, the patients received general anesthesia combined with epidural anesthesia.The anesthetic effect, heart rate (HR), systolic blood pressure (SBP), and diastolic blood pressure(DBP) were compared between the two groups.Results There were no significant differences in HR, SBP and DBP before anesthesia between the two groups (P＞0.05).The HR and SBP during intubation and extubation in the study group were lower than those in the control group, the differences were statistically significant (P＜0.05).There were no significant differences in HR and SBP between the two groups (P＞0.05).The SBP and DBP of the two groups 20 minutes after pneumoperitoneum were higher than those before anesthesia, the differences were statistically significant (P＜0.05).There were no significant differences in SBP and DBP between the two groups 20 min after deflation (P＞0.05).The SBP and DBP of 20 min after pneumoperitoneum in the study group were lower than those in the control group, the differences were statistically significant (P＜0.05).Conclusion General anesthesia combined with epidural anesthesia has a good clinical effect and high safety during radical surgery for rectal cancer.It is worthy of promotion and application in clinic.
