右美托咪定术前滴鼻用于腹股沟疝手术小儿镇静及预防术后躁动的临床效果

摘要目的探讨右美托咪定术前滴鼻用于腹股沟疝手术小儿的镇静及预防术后躁动的临床效果。方法选取我院2016年1月～2017年12月收治的60例行腹股沟疝手术的患儿，按照随机数字法分为对照组和研究组，每组各30例。对照组经鼻滴入生理盐水，研究组术前经鼻滴入右美托咪啶。比较两组用药后30 min的镇静评分、分离情绪评分、面罩抵抗评分、喉罩拔除时间、苏醒时间以及术后30 min、1、2 h时躁动评分。结果研究组用药后30 min的镇静评分显著低于对照组（P＜0.05）。研究组用药后30 min的分离情绪评分显著优于对照组（P＜0.05）。研究组用药后30 min的面罩抵抗评分显著高于对照组（P＜0.05）。两组喉罩拔除时间、苏醒时间的比较，差异无统计学意义（P＞0.05）。研究组术后30 min、1 h、2 h时躁动评分均显著低于对照组，差异均有统计学意义（P＜0.05）。结论行腹股沟疝手术的患儿术前经鼻滴入右美托咪啶具有良好的镇静作用，不影响苏醒质量且降低术后躁动发生率，值得在临床上推广使用。

Abstract：Objective To explore the clinical effect of the sedation and the prevention of restlessness in children with inguinal hernia before the operation of Dexmedetomidin for inguinal hernia surgery.Methods Selected 60 children undergoing inguinal hernia surgery in our hospital from January 2016 to December 2017 were divided into contron group and study group according to the random number table method,30 cases in each group.The control group were dripped into saline by nose,the study group were dripped into Dexmedetomidine through the nose before operation.The sedation score,separation emotion score,mask resistance score,laryngeal mask extraction time,recovery time,postoperative 30 min,1 h and postop 2 h,restlessnessscore of twogroupsof patients were compared between the two groups after 30 min treatment.Results The sedative score of 30 min in the study group was significantly lower than that of the control group,and the difference was statistically significant(P＜0.05）.The separation emotion score of 30 min in the study group was significantly higher than that of the control group(P＜0.05）.The masks resistance score of 30 min in the study group was significantly better than that of the control group,and the difference was statistically significant(P＜0.05）.There was no significant difference between the two groups of laryngeal mask extraction time and waking time(P＞0.05）.The restlessness scores of 30 min,1 h after operation and 2 h after operation in the study group were significantly lower than those in the control group (P<0.05）.Conclusion Preoperative Dexmedetomidine Drip through the nose has good sedative effect in children with inguinal hernia surgery.It does not affect the quality of awakening and reduce the incidence of postoperative agitation.It is worthy of clinical promotion.
