通圣方对急性脑卒中闭证神经功能缺损、便秘及hs-CRP的影响

摘要目的探讨通圣方对急性脑卒中闭证神经功能缺损、便秘及hs-CRP的影响。方法选取2016年3月～2018年3月在广东省中医药大学附属东莞市中医院脑病科或ICU住院的急性脑卒中闭症60例患者，将其随机分为两组，每组30例，在基础治疗基础上，治疗组加通圣方治疗，对照组加安宫牛黄丸治疗，疗程为7 d。观察两组治疗前后的便秘临床症状评分、神经功能缺损评分（NIHSS评分）及hs-CRP的变化情况。结果治疗后3、7 d，两组的便秘临床症状评分均有所下降，治疗组治疗后3、7 d的便秘临床症状评分明显低于对照组，差异有统计学意义（P<0.05）。两组治疗后7 d的NIHSS评分均低于治疗前，且治疗组治疗后7 d的NIHSS评分低于对照组，差异有统计学意义（P<0.05）。治疗组治疗后7 d的血清hs-CRP水平明显下降，与同组治疗前比较，差异有统计学意义（P<0.05）。结论通圣方能改善急性脑卒中闭症的神经功能缺损情况，推测作用机制可能与其改善患者便秘症状、减轻脑卒中后炎症损伤有关。

Abstract：bjective To investigate the influence of Tongsheng Prescription on neurological function deficits,constipation and hs-CRP for acute cerebral apoplexy with blocking syndrome.Methods All of 60 patients with acute cerebral apoplexy with blocking syndrome hospitalized in Department of Encephalopathy of Dongguan Hospital of Traditional Chinese Medicine Affiliated to Guangzhou University of Chinese Medicine or ICU from March 2016 to March 2018 were selected and they were randomly divided into the two groups and there were 30 patients in each group.On the basis of basic treatment,the treatment group was treated with Tongsheng Prescription while the control group was treated with Angong Niuhuang Pills.The course of treatment was 7 days.The neurological deficits,clinical symptom score of constipation and the change hs-CRP were observed before and after treatment in both groups.Results After treatment 3,7 d,clinical symptoms score of constipation in the two groups were decreased,and clinical symptoms score of constipation in the treatment group after treatment 3,7 d was significantly lower than the control group,and the differences were statistically significant(P<0.05).After treatment 7 d,NIHSS score in the two groups were lower than those before treatment,and NIHSS score in the treatment group after treatment 7 d was lower than that of the control group,and the difference was statistically significant(P<0.05).The serum hs-CRP level of after treatment 7 d in the treatment group was significantly decreased,compared with before treatment in the same group,the difference was statistically significant(P<0.05).Conclusion Tongsheng Decoction can improve the neurological function of deficits acute cerebral apoplexy with blocking syndrome,.It is speculated that the mechanism may be related to improving constipation symptoms and reducing inflammatory injury after cerebral apoplexy.
