Effect of Hospital-Community Integrated Management on blood glucose control in elderly patients with type 2 diabetes

Abstract:Objective To investigate the effect of hospital-community integrated management on blood glucose control in elderly patients with type 2 diabetes.Methods A total of 120 elderly patients with type 2 diabetes from September 2015 to September 2016 treated in our community health service center were selected as study subjects,and divided into the traditional group (n=60)and the integrated group (n=60)according to the random number table method.The patients in traditional group were treated by traditional hospital management.The patients in integrated group were treated with hospital-community integrated management mode.The blood glucose control effect,cognitive knowledge of diabetes knowledge and treatment compliance were compared between the two groups.Results After one year of management,the fasting blood glucose of patients in integrated group was lower than that of the traditional group at 2 h after meal.The difference was statistically significant(P<0.05).Cognitive score of diabetes knowledge and treatment compliance in the integrated group were higher than those in the traditional group (P<0.05).Conclusion The hospital-community integrated management model for elderly type 2 diabetes patients can improve the blood sugar control effect.
