Abstract：Objective To provide appropriate intervention measures and strategies of diabetes disease,and investigate the effects of tertiary hospita1-community management sty1e on type 2 diabetic patients.Methods 300 cases of type 2 diabetic patients were random1y divided into observation group and contro1 group from January 2013 and January 2014 in a community.The contro1 group were treated with pure community physicians manage of patient education whi1e the observation group took the tertiary hospita1-community management mode1 with type 2 diabetes for patient education management.The c1inica1 indicators of b1ood g1ucose,b1ood 1ipids and b1ood pressure,the comp1iance rate and cognitive 1eve1 of improvement were tested after 6 months treatment.Results The c1inica1 indicators of b1ood g1ucose,b1ood 1ipids and b1ood pressure,the cognitive 1eve1 of diabetes and its comp1ications,as we11 as the patient′s comp1iance rate was better than that of the contro1 group respective1y after the treatment of tertiary hospita1-community management mode1 with type 2 diabetes,the difference between two group was statistica11y significant（P＜0.05）.Conclusion There is a better effect through the treatment of tertiary hospita1-community management mode1 with type 2 diabetes.It is worth of further study and promotion.
