摘要目的评价二甲双胍联合甘精胰岛素治疗2型糖尿病的临床效果及安全性。方法 选取2014年1月～2015年12月于沈阳第四人民医院内分泌门诊就诊的符合纳入标准的60例患者，采用随机数字表法分为试验组和对照组各30例，对照组给予预混胰岛素治疗，试验组给予二甲双胍联合甘精胰岛素治疗，观察两组患者治疗3个月前后的空腹血糖（FBG）、餐后两小时血糖（2 h BG）、糖化血红蛋白（HbA1c）含量、体重指数（BMI）、胰岛素用量及发生低血糖事件情况。结果 治疗前，两组各观察指标比较，差异无统计学意义（P＞0.05）。治疗后，试验组FBG、2 h BG、HbA1c、BMI水平及胰岛素用量分别为 （6.41±0.93）mmol/L、（7.43±1.09）mmol/L、（5.72±1.53）%、（24.41±2.07）kg/m2、（9.13±3.27）U/d，对照组分别为（6.53±0.85）mmol/L、（8.75±1.41）mmol/L、（6.53±1.51）%、（26.31±2.11）kg/m2、（15.57± 4.21）U/d，治疗后组间FBG、2 h BG、HbA1c、BMI水平及胰岛素用量比较，差异有统计学意义（P＜0.05）。结论 相比应用预混胰岛素，二甲双胍联合基础胰岛素治疗2型糖尿病效果更佳更安全，值得临床推广应用。
Abstract：Objective To evaluate the clinical efficacy and safety of combination of Metformin and Insulin Glargine in the treatment of Type 2 diabetes mellitus.Methods Altogether 60 patients with type 2 diabetes mellitus who were treated in Department of Endocrinology of the Fourth People′s Hospital of Shenyang City from January 2014 to December 2015 and were in line with inclusion criteria were divided into experiment group and control group by random number table method,with 30 cases in each group.Control group was given Premixed Insulin treatment,while experiment group was treated with the combination of Metformin and Insulin Glargine.The fasting blood glucose(FBG),2-hour postprandial blood glucose(2 h BG),glycosylated hemoglobin(HbA1c),body mass index(BMI),insulin dosage and incidence of hypoglycemia events between two groups were observed before and after treatment for 3 months.Results All observation indicators before treatment between two groups showed no significant difference(P＞0.05);FBG,2 h BG,HbA1c,BMI level and insulin dosage after treatment for 3 months in the experiment group was(6.41±0.93)mmol/L,(7.43±1.09)mmol/L,(5.72± 1.53)%,(24.41±2.07)kg/m2and(9.13±3.27)U/d respectively,in contrast with in the control group of(6.53±0.85)mmol/L, (8.75±1.41)mmol/L,(6.53±1.51)%,(26.31±2.11)kg/m2and (15.57±4.21)U/d,with statistically significant difference between two groups(P＜0.05).Conclusion Compared with the application of Premixed Insulin,the combination of application of Metformin and Insulin Glargine in the treatment of type 2 diabetes mellitus is more effective and safe,which is worthy of clinical application.
