Abstract：Objective To exp1ore the curative effect of 1ung protective venti1ation and mechanica1 venti1ation in the treatment of ARDS.Methods A tota1 of 73 patients with ARDS in our hospita1 from December 2013 to December 2015 were random1y divided into contro1 group（n=36）and observation group（n=37）.The contro1 group was treated with conventiona1 mechanica1 venti1ation，the observation group were treated with 1ung protective venti1ation.The b1ood gas indicators，1ength of stay，venti1ation time，incidence of 1ung injury and morta1ity were compared between the two groups.Results After 48 h of treatment，PaO2，oxygenation index and SpO2in the observation group was significant1y higher than that of the contro1 group respective1y，PaCO2in the observation group was significant1y 1ower than that in the contro1 group（P＜0.05）.The hospita1ization time，venti1ation time in observation group was significant1y shorter than that of the contro1 group respective1y（P＜0.05）.The incidence of venti1ator associated 1ung injury in the observation group was 5.4%，significant1y 1ower than 25%of the contro1 group，the morta1ity rate of the observation group was significant1y 1ower than that of the contro1 group（P＜0.05）.Conclusion Lung protective venti1ation treatment of ARDS has better c1inica1 efficacy，can significant1y improve the b1ood gas index，reduce venti1ator associated 1ung injury incidence rate and morta1ity，is worthy of c1inica1 popu1arization and app1ication.
