甲氨蝶呤联合宫腔镜电切术治疗剖宫产瘢痕妊娠的效果

摘要目的探讨甲氨蝶呤联合宫腔镜电切术治疗剖宫产瘢痕妊娠的效果。方法选取2010年2月～2014年5月本院的72例被诊断为剖宫产瘢痕妊娠的患者作为研究对象，按照随机数字表法分成观察组（36例）和对照组（36例）。观察组给予甲氨蝶呤联合宫腔镜电切术治疗，对照组给予甲氨蝶呤联合清宫术进行治疗，比较两组的手术相关情况以及预后。结果观察组的出血量显著少于对照组，手术时间、血清β-HCG恢复正常时间以及月经恢复时间显著短于对照组，差异有统计学意义（P＜0.05）。观察组的总并发症发生率显著低于对照组，差异有统计学意义（P＜0.05）。观察组的妊娠率、顺产率显著高于对照组，差异有统计学意义（P＜0.05）。结论甲氨蝶呤联合宫腔镜电切术治疗剖宫产瘢痕妊娠的疗效显著，能促进患者恢复，明显减少并发症，预后良好，值得临床推广应用。

Abstract：Objective To explore the effect of methotrexate combined with hysteroscopic electric resection in the treatment of cesarean scar pregnancy.Methods 72 cases diagnosed as cesarean scar pregnancy from February 2010 to May 2014 in our hospital were selected and divided into the observation group and the control group according to the random number table method，with 36 cases in each group.The treatment group was given methotrexate combined with hysteroscopic electric resection，the control group was treated with methotrexate combined with curettage treatment.Surgery related condition and prognosis in the two groups was compared.Results The amount of bleeding in the observation group was less than that in the control group，the operation time，the recovery time and recovery time of serum β-HCG in the observation group was shorter than that in the control group，with significant difference　（P＜0.05）.The total incidence rate of complication in the observation group was lower than that in the control group，with significant difference （P＜0.05）.The pregnancy rate，the rate of vaginal delivery in the observation group was higher than that in the control group，with significant difference　（P＜0.05）.Conclusion The effect of methotrexate combined with hysteroscopic electric resection in the treatment of cesarean section scar pregnancy is significant，which can promote the recovery of patients，significantly reduce complication，with good prognosis，it is worthy of clinical promotion and application.
