Abstract:Objective To investigate the diagnosis and treatment strategy for huge primary retroperitoneal tumor(PRT). Methods Clinical data of 58 patientswith huge PRT treated from January 2010 to February 2014 in the First People′s Hospital of Yancheng City were retrospectively analyzed including clinicalmanifestation,image examination,tumor site,pathologic examination,operativemethod,follow-up and postoperative adjuvant therapy etc.Results Abdominalmass and pain were themost common clinical features.USG,CT and MRIare themainmeans in confirming the diagnosis of PRT. 20(34.5%)cases were benign tumors,38(65.5%)cases were malignant tumors,All 20 cases of benign tumor were resected completely.Of the 38 cases ofmalignant tumor,7(18.4%)cases underwent complete resection,23(60.5%)of the patients underwent combined resection with the involved organs that included colon,intestine,kidney,spleen,pancreas and bladder,respectively.6(15.8%)cases quasi-radical resection and 2(5.3%)cases palliative resection.15(39.5%)patients weremalignant tumors underwent reoperation due to recurrence.23 cases ofmalignant tumor patients received R0 resection,the duration of operations was (190±45)min,the volume of operative blood loss was (420±210)ml,and length of postoperative hospital stay ranged from 15 to 29 d.Operations were successfully performed in all the 58 patients.There was no perioperative death.The median follow-up period was(26±1.5)months,during which 1 case developed intra-abdominal lymph leakage and 1 case had hydrothorax or ascites.Conclusion Preoperative image examination,especially 3D-CT and MRI are crucial for the diagnosis of huge PRT,which are location,nature of the tumor and selection of operative modus.Completed resection and combined multiple organs resection is the essential procedure for the treatment of huge PRTs.
