Abstract:ObjectiveTo explore the efficacy of hospital-community self-management education model on patients with diabetes mellitus.Methods120 patients with diabetes mellitus in our hospital from September 2015 and September 2016 were selected and randomly divided into the observation group and the control group.Patients in the control group were given traditional education mode,while patients in the observation group were given hospital-community self-management education model.The levels of metabolic indexes,quality of life and cognition degree of diabetes knowledge were compared between two groups.ResultsBefore intervention,there were no significantly different among the levels of random blood glucose,total cholesterol,high-density lipoprotein cholesterol and glycosylated hemoglobin in two groups (P>0.05);after intervention,the levels of those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The scores of all indexes in quality of life and cognition degree of diabetes knowledge in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).ConclusionThe hospital-community self-management education pattern in patients with diabetes is effective,and it is worthy of clinical application.
