MRI与彩色超声联用应用对凶险型前置胎盘合并胎盘植入的诊断价值

摘要目的对比磁共振（MRI）与彩色超声联用或单独应用在凶险型前置胎盘合并胎盘植入诊断中的应用价值。方法选取2010年10月～2015年10月梅州市人民医院收治的70例疑为凶险型前置胎盘合并胎盘植入患者作为研究对象，均行经阴道超声检查及MRI检查，产后手术病理结果作为“金标准”，对比MRI单独使用、MRI联合彩色超声对凶险型前置胎盘合并胎盘植入诊断的敏感性、特异性及准确度。结果58例经病理检查诊断为凶险型前置胎盘合并胎盘植入，MRI单独使用对凶险型前置胎盘合并胎盘植入的诊断符合率为75.86%（49/70），MRI与彩色超声联用为93.10%（64/70），MRI与彩色超声联用检查对凶险型前置胎盘合并胎盘植入的诊断符合率显著高于MRI单独应用（χ2=10.324，P=0.001）。MRI单独使用对凶险型前置胎盘合并胎盘植入的诊断敏感性、特异性分别为75.86%（44/58）、41.67%（5/12）；MRI与彩色超声联用分别为93.10%（54/58）、83.33%（10/12），MRI与彩色超声联用诊断敏感性、特异性均显著高于MRI单独使用（χ2=6.576、4.444，P=0.010、0.045）。结论在凶险型前置胎盘合并胎盘植入的临床诊断中，MRI与彩色超声联用，结合声像图特征、临床表现、病史等进行综合分析，有助于降低误诊率，避免发生大出血、子宫破裂等严重后果

Abstract：ObjectiveTo compare the application value of MRI combined with color ultrasound or MRI alone in diagnosis of the dangerous type of placenta previa complicated with placenta accretes.MethodsSeventy suspected cases of pernicious placenta previa complicated with placenta accrete in Meizhou People′s Hospital from October 2010 to October 2015 were selected as the research object,all of whom were examined by ultrasound and MRI in vagina.The pathological results of postpartum surgery were used as a"gold standard".The diagnostic sensitivity,specificity and accuracy of using MRI alone and MRI combined with color Doppler ultrasound in diagnosis of the dangerous type of placenta previa complicated with placenta accreta.Results58 cases were confirmed by pathological examination in the diagnosis of pernicious placenta previa complicated with placenta accrete.The coincidence rate of using MRI alone in diagnosis of placenta previa complicated with placenta accreta was 75.86% (49/70).But the coincidence rate of MRI combined with color ultrasound in diagnosis of placenta previa complicated with placenta accreta was significantly higher than that of MRI alone (χ2=10.324,P=0.001).The diagnostic sensitivity and specificity rates of using MRI alone in diagnosis of placenta previa complicated with placenta accreta were 75.86%(44/58),41.67%(5/12).But diagnostic sensitivity and specificity rates of using MRI combined with color ultrasound in diagnosis of placenta previa complicated with placenta accreta were 93.10%(54/58),83.33%(10/12)respectively,which were significantly higher than those by using MRI alone(χ2= 6.576,4.444,P=0.010,0.045).ConclusionUsing MRI combined with ultrasound and comprehensive analysis of characteristics of ultrasonography,clinical manifestations and medical history in the clinical diagnosis of placenta previa complicated with placenta accreta is helpful to reduce the misdiagnosis rate and avoid the occurrence of bleeding,uterine rupture and other serious consequences.
