中药结肠透析对慢性肾脏病3～5期患者微炎症、营养状态的影响

摘要目的探讨中药结肠透析对慢性肾脏病（CKD）3～5期患者微炎症、营养状态的影响。方法选取2016年1月～2017年12月江西省九江市中医医院收治的60例CKD3～5期患者作为研究对象，按照随机数字表法将其分为对照组与治疗组，每组各30例。对照组患者采用中药灌肠治疗，治疗组患者采用中药结肠透析治疗，两组均治疗3个月。比较两组患者的炎症指标及营养状态。结果治疗3个月后，两组患者的血清超敏C-反应蛋白（hs-CRP）、白介素-6（IL-6）、肿瘤坏死因子-α（TNF-α）水平均低于治疗前，治疗组患者的血清hs-CRP、IL-6、TNF-α 水平均低于对照组，差异有统计学意义（P＜0.05）。治疗3个月后，两组患者的血浆清蛋白、转铁蛋白、血浆前清蛋白及血红蛋白水平均高于治疗前，治疗组患者的血浆清蛋白、转铁蛋白、血浆前清蛋白及血红蛋白水平均高于对照组，差异有统计学意义（P＜0.05）。结论中药结肠透析应用于CKD3～5期患者的治疗效果较好，可进一步减轻微炎症反应，改善营养状态。

Abstract：Objective To investigate the effect of traditional Chinese medicine colonic dialysis on the micro-inflammatory and nutrition status of patients with stage 3～5 chronic kidney disease (CKD).Methods A total of 60 patients with stage 3～5 CKD who were admitted to Jiujiang Hospital of Traditional Chinese Medicine in Jiangxi Province from January 2016 to December 2017 were selected as the research objects and divided into the control group and the treatment group according to the random number table method,with 30 cases in each group.The control group was treated with traditional Chinese medicine enema,while the treatment group was treated with traditional Chinese medicine colon dialysis,and both of the groups were treated for 3 months.The inflammation indicators and nutritional status of the two groups were compared.Results After 3 months of treatment,the levels of serum high-sensitivity C-reactive protein(hs-CRP),interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) in the two groups were lower than those before treatment,and the levels of hs-CRP,IL-6 and TNF-α in the treatment group were lower than those in the control group,the differences were statistically significant (P＜0.05).After 3 months of treatment,the levels of plasma albumin,transferrin,plasma prealbumin and hemoglobin in the two groups were higher than those before treatment,and the levels of plasma albumin,transferrin,plasma prealbumin and hemoglobin in the treatment group were higher than those in the control group,the differences were statistically significant (P＜0.05).Conclusion The traditional Chinese medicine colon dialysis is effective in the treatment of patients with stage 3～5 CKD,which can further reduce the micro-inflammatory reaction and improve the nutritional status.
