滋肾育胎丸结合枸橼酸氯米芬对不明原因不孕肾虚型患者子宫内膜容受性及妊娠结局的影响

摘要目的探讨滋肾育胎丸结合枸橼酸氯米芬对不明原因不孕肾虚型患者子宫内膜容受性及妊娠结局的影响。方法回顾性分析大连市妇幼保健院2017年1月～2019年1月收治的98 例肾虚型不明原因不孕患者的临床资料，根据治疗方法的不同进行分组，49 例采用枸橼酸氯米芬进行治疗的患者纳入对照组，49 例联合滋肾育胎丸进行治疗的患者纳入观察组。比较两组治疗前后子宫内膜容受性（包括内膜厚度以及A 型内膜形态比例）、卵巢储备功能[包括雌二醇（E2）、促卵泡激素（FSH）以及促黄体生成素（LH）水平]及妊娠结局（包括妊娠、活产及流产情况）。结果两组治疗后子宫内膜厚度及A 型内膜形态比例高于治疗前，差异有统计学意义（P＜0.05）；观察组治疗后子宫内膜厚度及A 型内膜形态比例高于对照组，差异有统计学意义（P＜0.05）。两组治疗后血清E2、FSH 及LH 水平高于治疗前，差异有统计学意义（P＜0.05）；观察组治疗后血清E2、FSH 以及LH 水平高于对照组，差异有统计学意义（P＜0.05）。观察组妊娠率及活产率高于对照组，流产率低于对照组，差异有统计学意义（P＜0.05）。结论滋肾育胎丸结合枸橼酸氯米芬促排卵治疗可调节肾虚型不明原因不孕患者子宫内膜容受性，改善其卵巢储备功能。

Abstract：Objective To investigate the effect of Zishen Yutai Pills combined with Clomifene Citrate on endometrial receptivity and pregnancy outcome in patients with kidney deficiency type of unexplained infertility. Methods The clinical data of 98 patients with kidney deficiency type of unexplained infertility who were treated in Dalian Maternal and Child Health Care Hospital from January 2017 to January 2019 were retrospectively analyzed. The patients were divided into groups according to different treatment methods, 49 patients with Clomifene Citrate were included in the control group, and 49 patients with Clomifene Citrate combined with Zishen Yutai Pills were included in the observation group. The endometrial receptivity (endometrial thickness and type-A endometrial morphology rate), ovarian reserve function (estradiol [E2], follicle-stimulating hormone [FSH], luteinizing hormone [LH]) and pregnancy outcome(preganncy, live birth, abortion) were compared between two groups before and after treatment. Results The endometrial thickness and type-A endometrial morphology rate in two groups after treatment were higher than those before treatment, and the differences were statistically sighificant (P＜0.05); after treatment, the endometrial thickness and type-A endometrial morphology rate in the observation group were higher than those in the control group, and the differences were statistically sighificant (P＜0.05). The levels of serum E2, FSH and LH in two groups after treatment were higher than those before treatment, and the differences were statistically sighificant (P＜0.05); after treatment, the levels of serum E2, FSH and LH in the observation group were higher than those in the control group, and the differences were statistically sighificant (P＜0.05). The pregnancy rate and live birth rate in the observation group were higher than those in the control group, and the abortion rate in the observation group was lower than that in the control group, and the differences were statistically significant (P＜0.05). Conclusion Zishen Yutai Pills combined with Clomifene Citrate for patients with kidney deficiency type of unexplained infertility can regulate endometrial receptivity and improve ovarian reserve function.
