ERCP及EST术中对患者进行体位干预对术中舒适性影响

摘要目的探讨术中体位干预在经内镜逆行性胰胆管造影术（ERCP）及内镜下十二指肠乳头括约肌切开术（EST）患者中的应用效果。方法选择2018年6月—2020年6月于南昌大学第四附属医院行ERCP+EST 手术治疗的108 例患者作为研究对象，采用随机数字表法分为对照组（54 例）和观察组（54 例）。对照组采取常规干预，观察组在此基础上实施体位干预。比较两组术前和术后6 h 的舒适度[舒适状况量表（GCQ）]评分，记录并比较两组的手术情况（术前体位安置时间、手术时间和术后住院时间），统计并比较两组的并发症发生情况。结果观察组干预后的GCQ 评分高于对照组，差异有统计学意义（P＜0.05）。观察组的术前体位安置时间、手术时间、术后住院时间均短于对照组，差异有统计学意义（P＜0.05）。观察组的并发症总发生率低于对照组，差异有统计学意义（P＜0.05）。结论 ERCP及EST术中对患者进行体位干预能够提高患者舒适性，降低体位相关并发症发生风险，有助于促进患者术后康复。

Abstract：Objective To explore the application effect of intraoperative postural intervention in patients with endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic sphincterotomy (EST).Methods A total of 108 patients who underwent ERCP+EST surgical treatments in the Fourth Affiliated Hospital of Nanchang University from June 2018 to June 2020 were selected as the research subjects.They were divided into the control group (54 cases)and the observation group(54 cases)by the random number table method.In the control group,routine intervention was used,and in the observation group, postural intervention was added on the basis of the control group.The general comfort questionnaire(GCQ) scores before surgery and 6 h after surgery were compared in the two groups.The surgical operations such as preoperative posture arrangement time, operation time, and postoperative hospital stay were recorded.The incidence of complications in the two groups was calculated and compared.Results The GCQ score of the observation group after intervention was higher than that of the control group, and the difference was statistically significant (P＜0.05).In the observation group, the preoperative posture arrangement time, operation time, and postoperative hospital stay were shorter than those in the control group, and the differences were statistically significant (P＜0.05).The total incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P＜0.05).Conclusion Postural intervention for patients during ERCP and EST surgery can improve patient comfort, reduce the risk of posture-related complications, which will help to promote postoperative recovery.
