Abstract:Objective To analyze the application effect of ultrasound-guided femoral nerve and lateral femoral cutaneous nerve block in elderly patients undergoing hip replacement.Methods A total of 64 patients who underwent hip arthroplasty in Dandong Central Hospital of Liaoning Province from April 2017 to April 2018 were selected as the research objects.According to the random number table method, they were divided into the control group and the observation group, with 32 cases in each group.The control group was treated with simple intubation general anesthesia during the operation, while the observation group was treated with B-ultrasound-guided femoral nerve and lateral femoral cutaneous nerve block anesthesia during the operation.Before induction anesthesia (T0), skin incision (T1), 30 minutes after operation (T2), at the end of operation (T3) and 1 hour after operation (T4), the stress response indexes such as interleukin-6 (IL-6), blood glucose (Glu) and cortisol (Cor), as well as vital signs such as heart rate(HR), systolic blood pressure (SBP) and diastolic blood pressure (DBP) and stress response were compared between the two groups.Results The levels of HR, SBP and DBP at T1, T2, T3 and T4 in the control groups were lower than those at T0, and the differences were statistically significant (P<0.05).The levels of HR, SBP and DBP at T1, T2, T3 and T4 in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05).However,there was no significant difference in the levels of HR, SBP and DBP at T0 between the two groups (P>0.05).The levels of IL-6, Glu and Cor at T1, T2, T3 and T4 in the control groups were higher than those at T0, and the differences were statistically significant (P<0.05).The IL-6, Glu and Cor levels at T1, T2, T3 and T4 in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.05).However, there was no significant difference in IL-6, Glu and Cor levels between the two groups at T0 (P>0.05).Conclusion B-ultrasound-guided femoral nerve and lateral femoral cutaneous nerve block in elderly patients with hip replacement surgery can obtain better anesthetic effect, maintain the patient′s stress response, and maintain a more stable level of vital signs.Therefore, it is especially suitable for elderly people with poor physical tolerance and poor autoimmune function, which is worthy of promotion and use.
