Study on the correlation between quantification of gout urate deposition and blood uric acid level by energy spectrum CT

Abstract:Objective To study the correlation between the quantitative determination of urate deposition and blood uric acid level by energy spectrum CT.Methods Sixty-eight patients with chronic gout treated in our hospital from January 2018 to January 2019 were recruited in the gout group,and another 68 patients with asymptomatic and nonimaging hyperuricemia who were treated in our hospital during the same period were selected as the reference group.All patients were performed with energy spectrum CT examinations.The energy spectrum CT examinations of two groups were compared,such as calcium urate concentration in the soft tissue adjacent to the first metatarsophalangeal joint,bone cortex,cancellous bone and blood uric acid concentration,the correlation was analyzed.The changes in the soft tissues adjacent to the first metatarsophalangeal joint,the total volume of uric acid crystals in the feet and ankles,and themaximum individual volume of uric acid crystals in patients with gout before and after treatmentwere observed.Results The level of blood uric acid and the concentration of calcium urate in the soft tissue adjacent to the first metatarsophalangeal joint in the gout group were higher than those in the reference group,and the differences were statistically significant(P<0.05).There were no significant differences in the calcium urate concentrations of cancellous bone and cortical bone between the two groups of patients(P>0.05).There was amoderate positive correlation between blood uric acid level and calcium urate concentration in the soft tissue adjacent to the first metatarsophalangeal joint(r=0.703,P<0.05),and therewas no significant correlation with calcium urate concentration in cancellous bone and cortical bone(r=-0.005,r=-0.067,P>0.05).The blood uric acid level of patients with gout was highly positively correlated with the total volume of uric acid crystals in the feet and ankles and the maximum individual volume of uric acid crystals(r=0.981,r=0.990,P<0.05).After treatment of gout patients,the concentration of calcium urate in the soft tissue adjacent to the first metatarsophalangeal joint,the total volume of uric acid crystals in the feet and ankles,and the maximum individual volume of uric acid crystals were lower than those before treatment,and the differences were statistically significant(P<0.05).Conclusion Energy spectrum CT test can confirm that there is definitely positive correlation between urate deposition in gout patients and the concentration of calcium urate in the soft tissue adjacent to the first metatarsophalangeal joint and blood uric acid level,and both indicators are reversible and can be improved after regular treatment.
