摘要目的 探讨无创助产分娩改善产妇盆底功能和产后恢复的应用效果。方法 选取2017年8月~2018年3月在我院分娩单胎妊娠足月初产妇168例为研究对象,采用随机数字表法将其分为观察组和对照组,每组各84例。对照组给予传统助产,观察组给予无创助产。比较两组产后24 h 出血量,伤口疼痛评分、住院时间、产后42 d 盆底肌受损(含Ⅰ、Ⅱ类肌纤维)、压力性尿失禁和产后3个月性功能障碍发生情况。结果 观察组产后出血量少于对照组,伤口疼痛评分低于对照组,住院时间短于对照组,压力性尿失禁和性功能障碍发生率低于对照组,差异均有统计学意义(P<0.05)。两组产后Ⅰ、Ⅱ类肌纤维受损率比较,差异无统计学意义(P>0.05)。结论 较传统助产分娩,无创助产可降低产妇产后出血量,减轻伤口疼痛,缩短住院时间,促进产后恢复,值得在产科分娩推广应用。
Abstract:Objective To explore the application effect of noninvasive delivery on improving the pelvic floor function and postpartum recovery.Methods A total of 168 full-term primipara with single pregnancy delivered in our hospital from August 2017 to March 2018 were selected as research subjects.The parturients were divided into observation group and control group according to random number table method, 84 cases in each group.The control group was given traditional midwifery, while the observation group was given noninvasive midwifery.The 24-hour postpartum bleeding volume, wound pain score, hospitalization time, pelvic floor muscle damage (including type Ⅰand type Ⅱmuscle fibers), stress urinary incontinence and 3-month postpartum sexual dysfunction were compared between the two groups.Results The amount of postpartum hemorrhage in the observation group was less than that in the control group, the wound pain score was lower than that in the control group, the hospitalization time was shorter than that in the control group, the incidence of stress urinary incontinence and sexual dysfunction were lower than that in the control group,and the differences were statistically significant (P<0.05).There were no significant differences in the rates of typeⅠand type Ⅱmuscle fibers damage between the two groups (P>0.05).Conclusion Compared with traditional midwifery,non-invasive midwifery can reduce postpartum hemorrhage, reduce wound pain, shorten hospital stay, and promote postpartum recovery.It is worth popularizing in obstetric delivery.
