系统化护理干预对喉癌术后鼻饲患者营养状况及心理状态的影响

摘要目的探讨系统化护理干预对喉癌术后鼻饲患者营养状况及心理状态的影响。方法选取2017年12月～2019年12月我院收治的92 例喉癌患者，按随机数字表法分为两组，每组各46 例。对照组采取常规护理，观察组实施系统化护理干预，比较两组干预前、干预7 d 后的营养状况[血红蛋白（Hb）、血清清蛋白（ALB）、前清蛋白（PA）]，并采用焦虑自评量表（SAS）和抑郁自评量表（SDS）评估患者心理状态。结果两组干预前Hb、ALB、PA 及SAS、SDS 评分比较，差异无统计学意义（P＞0.05）；观察组干预后Hb 为（135.67±22.54）g/L、ALB 为（42.57±6.53）g/L、PA 为（235.37±35.528）mg/L，高于对照组的（111.53±21.54）g/L、（32.57±6.52）g/L、（199.87±28.54）mg/L，差异均有统计学意义（P＜0.05）；观察组干预后SAS 评分为（24.56±4.26）分，SDS 评分为（28.97±8.52）分，低于对照组的（34.67±7.52）分、（36.57±5.24）分，差异均有统计学意义（P＜0.05）。结论系统化护理干预能改善喉癌术后鼻饲患者营养状况，缓解患者焦虑、抑郁等负面情绪。

Abstract：Objective To investigate the effect of systematic nursing intervention on nutritional status and psychological status of postoperative patients with nasal feeding after laryngeal cancer surgery.Methods A total of 92 patients with laryngeal cancer treated in our hospital from December 2017 to December 2019 were selected and divided into two groups according to the random number table method, 46 cases in each group.The control group received routine nursing and the observation group received systematic nursing intervention.The nutritional status of hemoglobin (Hb),serum albumin(ALB) and prealbumin (PA) before and after 7 days intervention were compared between the two groups,self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were used to assess the mental status of patients.Results There were no significant differences in Hb, ALB, PA, SAS, and SDS scores between the two groups before intervention (P＞0.05).The Hb for (135.67±22.54) g/L, ALB for (2.57±6.53) g/L, and PA for (235.37±35.528) mg/L in the observation group after intervention were higher than (111.53±21.54) g/L, (32.57±6.52) g/L, (199.87±28.54) mg/L in the control group, the differences were statistically significant (P＜0.05).The SAS score of the observation group after the intervention was (24.56±4.26) points and the SDS score was (28.97±8.52) points, which were lower than the control group′s (34.67±7.52) points and (36.57±5.24) points, with statistical differences (P＜0.05).Conclusion Systemic nursing intervention can improve the nutritional status and relieve anxiety, depression and other negative emotions of patients with nasal feeding after laryngeal cancer.
