Abstract：Stroke is a frequent and common disease, with a high rate of disability and a long course of illness, which seriously affects the ability to survive and quality of life of patients.Continuity nursing has a certain role in the recovery of daily life ability, medical compliance, and nursing satisfaction of patients with stroke.This paper reviews the status quo, development direction and trend of the continuous care model in stroke patients in China, as well as the final effect evaluation.The model of continuous care has changed from a simple hospital-patient to hospital-community-patient-family model, and the use of community resources has effectively landed the model of continuous care.On the other hand, with the development of information technology, it has changed from simple telephone and door follow-up to informatization follow-up.The application of this model has been improved and widely used by the use of the aging advantage of information technology.Stroke patients who use the continuous care model have improved their self-care ability, compliance, quality of life, and satisfaction.It can improve the mental condition of the patient and reduce the social pressure and economic burden of the patients′family.At the same time, this paper summarizes the problems and suggestions in the implementation of continuous nursing mode in stroke patients.It provides the basis for the practice of continuous nursing mode of stroke.The implementation process of the continuous care model is not perfect enough to be replicable.At the same time, the use of community resources needs relevant policy support, and it is also one of the shortcomings of this model.
