中西医结合治疗糖尿病周围神经病变的效果及对血清同型半胱氨酸和胱抑素C水平的影响

摘要目的探索中西医结合治疗糖尿病周围神经病变（DPN）患者疗效及对血清同型半胱氨酸（Hcy）和胱抑素C（Cys-C）水平的影响。方法选取2015年10月～2018年6月在我院就诊的DPN患者124例，随机分为实验组和对照组，每组各62例。在基础药物治疗的基础上，对照组每日给予1次500 μg甲钴胺注射液点滴治疗。实验组则在对照组的基础上联合使用中药芪参汤治疗。2个疗程后，比较两组的多伦多临床评分（TCSS）、感觉神经传导速度（SNCV）、运动神经传导速度（MNCV）以及Hcy、Cys-C水平，以评价传统西医基础治疗方案加用中药芪参汤治疗糖尿病DPN的治疗效果。结果经过2个疗程治疗后，实验组的治疗总有效率高于对照组，差异有统计学意义（P＜0.05）；实验组治疗后的TCSS评分低于对照组，差异有统计学意义（P＜0.05）；实验组治疗后的MNCV、SNCV均高于对照组，差异有统计学意义（P＜0.05）；实验组治疗后的Hcy和Cys-C水平均低于对照组，差异有统计学意义（P＜0.05）。结论中西医结合疗法对DPN患者具有显著的效果，该疗法可显著降低血清中Hcy和Cys-C含量。

Abstract：Objective To explore the effect of integrated traditional Chinese and western medicine on diabetic peripheral neuropathy(DPN)patients and levels of homocysteine(Hcy)and cystatin-C(Cys-C)in serum.Methods A total of 124 DPN patients in our hospital from October 2015 to June 2018were selected and randomly divided into experimental group and control group,62 cases in each group.On the basis of basic drug treatment,the patients in control group were given 500 μg Mecobalamin Injection once a day.The experimental group was treated with Huangqi Xuanshen Decoction on the basis of the control group.After two courses of treatment,toronto clinical score(TCSS),sensory nerve conduction velocity(SNCV),motor nerve conduction velocity(MNCV)and levels of HCY and Cys-C were compared between the two groups,in order to evaluate the therapeutic effect of traditional western medicine basic treatment plus Huangqi Xuanshen Decoction on diabetic peripheral neuropathy.Results After two courses of treatment,the total effective rate of the experimental group was higher than that of the control group(P<0.05）;the TCS score of the experimental group was lower than that of the control group(P<0.05）;the NCV and SNCV of the experimental group were higher than those of the control group(P<0.05）;the levels of HCY and Cys-C of the experimental group were lower than those of the control group,and the differences were statistically significant(P<0.05）.Conclusion The combination of traditional Chinese medicine and western medicine has a significant effect on DPN patients.Moreover,this therapy can significantly reduce the levels of HCY and Cys-C in serum.
