全程心理护理干预在腰硬联合麻醉患者中的应用效果

摘要目的探讨全程心理护理干预应用行腰硬联合麻醉患者的临床效果。方法选取2015年2月～2016年2月在我院行腰硬联合麻醉的手术患者120例作为研究对象，采用随机数字表法将其分为对照组、观察组，每组60例。对照组患者采用常规护理，观察组患者采用全程心理护理干预。比较两组患者手术中的应激反应及患者满意度情况。结果干预前，两组患者的收缩压和心率指标比较，差异无统计学意义（P＞0.05）；干预后，两组患者的收缩压和心率指标均高于干预前；且观察组患者的收缩压和心率的指标低于对照组，差异均有统计学意义（P＜0.05）。干预前，两组患者的肾上腺素与去甲肾上腺素浓度指标变化的比较，差异无统计学意义（P＞0.05）；干预后，两组患者的肾上腺素与去甲肾上腺素浓度的比较，均高于干预前；且观察组患者的肾上腺素与去甲肾上腺素浓度指标低于对照组，差异均有统计学意义（P＜0.05）。观察组患者的护理总满意度显著高于对照组，差异有统计学意义（P＜0.05）。结论对行腰硬联合麻醉的手术患者实施全程心理护理干预可以减轻患者在围术期的应激反应，有利于患者的生命体征保持稳定，并能提高患者的满意度。

Abstract：Objective To explore the clinical effect of whole course psychological nursing intervention on patients with combined spinal and hard anesthesia.Methods A total of 120 surgical patients who underwent combined spinal and epidural anesthesia in our hospital from February 2015 to February 2016 were selected as research objects.They were divided into control group and observation group with 60 cases in each group by random number table method.Patients in the control group were given routine nursing while patients in the observation group were given psychological nursing intervention in the whole course.The stress response and patient satisfaction during the operation were compared between the two groups.Results Before the online translation intervention,there was no significant difference between the two groups in systolic blood pressure and heart rate(P＞0.05).After the intervention,the systolic blood pressure and heart rate indexes of the two groups were higher than those before the intervention.Moreover,the indexes of systolic blood pressure and heart rate in the observation group were lower than those in the control group,and the difference was statistically significant(P＜0.05).Before the intervention,there was no significant difference between the two groups(P＞0.05).After the intervention,the concentrations of adrenaline and norepinephrine in the two groups were higher than those before the intervention.Moreover,the concentrations of epinephrine and norepinephrine in the observation group were lower than those in the control group,and the difference was statistically significant(P＜0.05).The total nursing satisfaction of patients in the observation group was significantly higher than that in the control group (P＜0.05).Conclusion the whole psychological nursing intervention for patients undergoing combined spinal epidural anesthesia can relieve stress response during perioperative period,maintain stability of vital signs and improve patient satisfaction.
