The correlation between drug resistance of Klebsiella pneumoniae and antibacterial use intensity in patients with hematological diseases

Abstract:ObjectiveTo analyze the correlation between drug resistance of Klebsiella pneumonia and antibacterial use density(AUD)in hospitalized patients with hematological diseases,so as to provide reference for clinical rational use of antibacterial drugs.MethodsA retrospective analysis was used to the drug resistance rate of Klebsiella pneumonia and AUD of frequently-used antibacterial drugs for hospitalized patients from 2013 to 2017 in the Institute of Hematology and Blood Disease Hospital,Chinese Academy of Medical Sciences.The correlation between drug resistance and AUD was analyzed using the Spearman correlation method.ResultsThe drug resistance rates of Klebsiella pneumonia to Cefepime and Meropenem were significantly positively correlated to the AUD of the fourth generation cephalosporins and carbapenem drugs(Cefepime r=0.784,P<0.01;Meropenem r=0.894,P<0.01),respectively.No significant correlation could be found between the drug resistance rates of Klebsiella pneumonia to other antimicrobial drugs and AUD of the relevant sorts of bacterial drugs.Meanwhile,the fourth generation cephalosporins,carbapenems,and Tigecyclines were significantly positively correlated with Klebsiella pneumonia resistance to various bacterial drugs.ConclusionThe resistance rates of Klebsiella pneumonia has a certain correlation with AUD of antibacterial drugs.It is recommended that clinical use of the fourth generation cephalosporins,carbapenems,and Tigecycline for the treatment of Klebsiella pneumonia should be particularly careful in order to control and delay the growth of resistant bacteria.
