富血小板血浆与透明质酸钠治疗膝骨关节炎的效果比较

摘要目的比较富血小板血浆（PRP）及透明质酸钠（SH）治疗膝骨关节炎的临床效果。方法选取2016年3月～2017年5月在我院住院的80例膝骨关节炎患者，按照随机数字表法分为PRP组和SH组，每组各40例。PRP组行PRP关节腔注射，SH组行SH关节腔注射，均每周1次，共5次。两组均配合蜡疗、微波治疗及超声波治疗。于治疗前、治疗后、治疗后3、6个月随访并应用膝骨关节炎评分量表（western ontario and mcmaster universities osteoar thritis index，WOMAC）及视觉模拟评分法（visual analogue score，VAS）进行疗效评估，治疗前及治疗后6个月行膝关节磁共振检查比较影像学结果。结果在WOMAC评分及VAS评分方面，两组治疗后均低于治疗前，差异有统计学意义（P＜0.05）；治疗结束后、治疗后3、6个月比较，PRP组低于SH组，差异均有统计学意义（P＜0.05）。磁共振检查结果显示，PRP组好转率高于SH组，差异有统计学意义（P＜0.05）。结论PRP与SH治疗膝骨关节炎均有较好的效果，但PRP疗效更加显著。

Abstract：ObjectiveTo compare the clinical effects of platelet-rich plasma(PRP)and sodium hyaluronate(SH)in the treatmentof knee osteoarthritis.MethodsA total of 80 patientswith knee osteoarthritiswhowere hospitalized in our hospital from March 2016 toMay 2017were divided into PRPgroup and SH group according to random number table,40 cases in each group.PRP group was injected with PRP in the joint cavity,and the SH group was treated with SH in the joint cavity.Theywere performed once aweek for 5 times.Both groupswere provided with wax,microwave,and ultrasound therapies.Thewestern ontario andmcmaster universities osteoarthritis index scale(WOMAC)and visual analogue scale(VAS)were evaluated for effects before treatment,just after therapy,and 3 and 6months after treatment during follow up visits.The radiographic results of knee joints using magnetic resonance imaging were compared before and 6 months after treatment.ResultsTheWOMAC score and VASscorewere significantly lower in the two groups after treatment than those before treatment(P＜0.05).Thescores in the PRPgroupwas lower than those in the SH group justafter therapy,3 and 6months after treatment,with statistical differences(P＜0.05).The improvement rate in the PRP group was better than that in the SH group aftermagnetic resonance imaging detection,with a significant difference(P＜0.05).ConclusionBoth PRP and SH are effective in the treatment of knee osteoarthritis,and the effect in the PRP ismore remarkable.
