胸腹腔镜联合手术治疗老年食管癌的临床效果及安全性观察

摘要目的探讨胸腹腔镜联合手术治疗老年食管癌的近期效果与安全性。方法选择我院2015年1月～2016年4月收治的76例老年食管癌患者作为研究对象，随机分为观察组与对照组，各38例。对照组接受常规开胸食管癌根治术治疗，观察组接受胸腹腔镜联合食管癌根治术治疗，比较两组的手术情况、并发症发生情况及预后情况。结果两组的淋巴结清扫数目、手术时间等指标比较，差异无统计学意义（P＞0.05）。观察组的术中出血量少于对照组，术后拔管时间短于对照组，差异有统计学意义（P＜0.01）。观察组的术后并发症发生率为13.16%，低于对照组的28.95%，差异有统计学意义（P＜0.05）。观察组的术后淋巴转移率、再次手术率、术后6及9个月累计死亡率分别为5.26%、2.63%、2.63%、5.26%，与对照组的2.63%、0.00%、0.00%、2.63%比较，差异无统计学意义（P＞0.05）。结论老年食管癌应用胸腹腔镜联合手术治疗效果确切，创伤小，术后恢复快，并发症发生率较低，预后良好，值得推广。

Abstract：Objective To investigate the short-term efficacy and safety of thoracoscopy and laparoscopy combined with surgery in the treatment of elderly patients with esophageal cancer.Methods 76 elderly patients with esophageal cancer treated in our hospital from January 2015 to April 2016 were selected as the research object and were randomly divided into the observation group and the control group,38 cases in each group.Patients in the control group were treated with conventional open thoracic esophageal cancer radical surgery while patients in the observation group were treated with thoracic laparoscopic surgery combined with esophageal cancer radical surgery.The condition of operation,complications and prognosis of the two groups were compared.Results There were no significant differences in the number of lymph node dissection and operation time between the two groups(P＞0.05).The amount of intraoperative bleeding in the observation group was less than that in the control group,and the extubation time in the observation group was shorter than that in the control group,with significant differences(P＜0.01).The incidence rate of complication in the observation group after operation was 13.16%,which was lower than that in the control group(28.95%),with significant difference(P＜0.05).The postoperative lymph node metastasis rate,reoperation rate,postoperative 6 and 9 months cumulative mortality rate were 5.26%,2.63%,2.63%,5.26%in the observation group,compared with the control group (2.63%,0.00%,0.00%, 2.63%),the differences were not statistically significant(P＞0.05).Conclusion The effect of the application of thoracoscopy and laparoscopy combined with surgery in the treatment of elderly patients with esophageal cancer is definite and the wound is small and the incidence of postoperative complications is low with good prognosis,it is worth promoting.
