C反应蛋白和降钙素原在前列腺增生合并尿路感染早期诊断中的临床意义

摘要目的研究C-反应蛋白（CRP）和降钙素原（PCT）在前列腺增生（BPH）合并尿路感染（UTI）早期诊断中的意义。方法对2014年3月～2017年4月在我院住院明确诊断为前列腺增生的患者筛选合并UTI患者77例，根据尿细菌学培养结果分为感染组（49例）和对照组（28例）。评价CRP和PCT对诊断BPH合并UTI的特异度、敏感度。结果感染组和对照组CRP和PCT水平差异有统计学意义（P<0.01）；CRP和PCT水平在BPH合并UTI诊断中的ROC曲线下面积分别为0.805和0.859；对诊断BPH合并UTI的敏感度分别为89.7%、87.6%，特异度分别为60.7%、71.4%；血清CRP和PCT水平有相关性（r=0.509，P<0.01）。结论血清CRP和PCT可作为诊断BPH合并UTI的有效指标，可早期发现BPH合并UTI情况。

Abstract：ObjectiveTo study the early diagnostic value of C-reactive protein(CRP)and procalcitonin(PCT)in urinary tract infection(UTI)in patients with benign prostatic hyperplasia(BPH).MethodsA prospective study was performed on 77 patients with suspected UTI with BPH in our hospital.According to the urine bacterial culture results,77 cases were divided into infection group(n=49)and control group(n=28).The sensitivity and specificity of CRP and PCT in the diagnosis of UTI patients with BPH,and their correlation were evaluated.ResultsThere was significant difference in CRP and PCT levels between two groups(P<0.01).The area under ROC curve was 0.805 for CRP and 0.859 for PCT respectively in UTI patients with BPH.The sensitivity and specificity of CRP and PCT in the diagnosis of UTI patients with BPH were 89.7%and 87.6%,and 60.7%and 71.4%,respectively.The correlation efficient between CRP and PCT was 0.509(P<0.01).ConclusionSerum levels of CRP and PCT increase in UTI patients with BPH,the detection of serum CRP and PCT levels is helpful to early diagnosis the UTI patients with BPH.
